王东教授治疗糖尿病视网膜病变的经验总结

【摘要】：目的:吾师王东教授从事内分泌临床工作及科研十余载,对如何运用中医药治疗糖尿病视网膜病变有其独到的见解及诊疗方式。本论文旨在总结吾师治疗糖尿病视网膜病变的临床经验,希望为临床治疗糖尿病视网膜病变提供一些新的启示,以期在临床应用中为患者解除痛苦。方法:通过跟随吾师王东教授学习,结合阅读并分析近年来关于糖尿病视网膜病变的有关文献,从中医的角度对糖尿病视网膜病变的病名、病因病机及治疗进行阐述,总结其治疗糖尿病视网膜病变的临床经验。结论:1.吾师在总结古人及现代各医家理论基础下认为从中医角度糖尿病视网膜病变应命名为消渴视衣病;2.吾师依据中医审证求因的理论,从“脾主运化”论治消渴的观点出发,认为本病的病位在脾、肝、肾;其中非增殖期糖尿病视网膜病变以脾虚、气血不足为基本病机;增殖期糖尿病视网膜病变以肝肾阴虚、肾精不足为基本病机;而且血瘀贯穿于疾病的始终;3.吾师在中医治病求本理论的指导下,结合糖尿病视网膜病变分期对其进行辨证论治。在临床治疗上非增殖期糖尿病视网膜病变以健脾固摄,益气补血、化瘀止血为基本治疗原则,以归脾汤合四物汤加减治疗;增殖期糖尿病视网膜病变以滋肾养肝明目,补肾益精,化瘀止血为基本治疗原则,以杞菊地黄丸合桃红四物汤加减治疗本病。
[Abstract]:Objective: Professor Wang Dong has engaged in endocrine clinic work and scientific research for more than ten years, on how to use Chinese medicine for the treatment of diabetic retinopathy with his insights and treatment. This paper aims to summarize my clinical experience in the treatment of diabetic retinopathy, hoping to provide some inspirations for the clinical treatment of diabetic retinopathy, in order to relieve pain patients in the clinical application. Methods: by following my teacher professor Wang Dong learning, by reading and analysis of related literature about diabetic retinopathy in recent years, from the perspective of Chinese medicine on diabetic retinopathy disease, pathogenesis and treatment are described, summarize the clinical experience in the treatment of diabetic retinopathy. Conclusion: 1. in love based on summarizing the modern theory and the doctors think that diabetic retinopathy from the viewpoint of traditional Chinese medicine should be named as Xiaoke garment 2. according to the Chinese medicine disease; my request for the trial evidence theory on the treatment of diabetes in the view of "spleen transport", considered that the disease is located in the spleen, liver and kidney; the non proliferative diabetic retinopathy with spleen deficiency, blood deficiency is the basic pathogenesis; proliferative diabetic retinopathy in the liver and kidney yin deficiency, kidney essence deficiency is the basic pathogenesis of the disease; and blood stasis throughout the beginning; 3. teacher treat the theory in traditional Chinese medicine, combined with the stage of diabetic retinopathy. The syndrome differentiation and treatment in the clinical treatment of non proliferative diabetic retinopathy with Jianpi Gushe, Qi and blood, removing blood stasis and hemostasis for the basic principle of treatment, with Guipi Decoction and Siwu Decoction treatment; proliferative diabetic retinopathy with nourishing kidney and Liver eyesight, kidney essence, blood hemostasis as the basic treatment principle, Qiju Dihuang Pill Taohong Siwu Decoction in the treatment of this disease.
