腰痛息汤内服热熨配合功能锻炼治疗寒湿型腰椎间盘突出症的临床观察

【摘要】：目的根据临床腰痛息汤内服热熨配合功能锻炼治疗寒湿型腰椎间盘突出症,观察患者在治疗前后的综合疗效,探讨腰痛息汤对改善寒湿型腰椎间盘突出症下腰痛的作用机理,为临床在治疗此病寒湿型的方案选择中提供依据。方法严格按照病例选入和排除的标准,于2014年5月至2015年12月实习期间,观察了宜昌市中医医院骨伤科的符合纳入标准的120例该病患者。按治疗方案的不同,将患者分为三组,即治疗组、对照1组、对照2组。治疗组接受腰痛息汤内服热熨,并配合功能锻炼和健康宣教及心理沟通治疗;对照1组接受海桐皮汤熏洗,并配合功能锻炼和健康宣教及心理沟通治疗;对照2组接受腰痛息汤口服,并配合功能锻炼和健康宣教及心理沟通治疗。一周为一个疗程,治疗4周后,评定三组的综合疗效。结果总体疗效表明:治疗组痊愈19例,好转20例,无效3例,总有效率达92。9%;对照1组治愈13例,好转23例,无效4例,总有效率为90.0%;对照2组治愈11例,好转22例,无效5例,总有效率为86.8%。治疗组疗效明显优于对照1组和对照2组。治疗组与两对照组总有效率比较差异有统计学意义(P0.05)。结论腰痛息汤内服热熨结合功能锻炼治疗寒湿型腰椎间盘突出症的方法疗效显著。此治疗方法采用中医保守治疗的方法,具有无创伤、操作简易方便、价格优惠等特点,在临床具有推广价值。
[Abstract]:Objective according to the clinical treatment of cold and wet lumbar intervertebral disc herniation with hot ironing and functional exercise, to observe the comprehensive curative effect of the patients before and after treatment, and to explore the mechanism of improving the lower back pain of cold and wet lumbar intervertebral disc herniation. Methods according to the criteria of case selection and exclusion, the clinical practice was conducted from May 2014 to December 2015. 120 patients of Yichang traditional Chinese Medicine Hospital according to the inclusion criteria were observed. According to the different treatment schemes, the patients were divided into three groups: treatment group, control group 1, control group 2. In combination with functional exercise, health education and psychological communication treatment, the control group was treated with Haitongpi decoction fumigation and washing, and with functional exercise and health education and psychological communication treatment. Combined with functional exercise, health education and psychological communication, a course of treatment was conducted once a week. After 4 weeks of treatment, the comprehensive efficacy of the three groups was evaluated. Results the overall curative effect showed that 19 cases were cured, 20 cases were improved, and 3 cases were ineffective in the treatment group. The total effective rate was 92.9%; in the control group, 13 cases were cured, 23 cases were improved, 4 cases were ineffective, and the total effective rate was 90.00.The control group 2 was cured in 11 cases, improved in 22 cases, and failed in 5 cases. The total effective rate was 86.8. The curative effect of the treatment group was obviously better than that of the control group 1 and the control group 2. The difference between the total effective rate of the treatment group and the two control groups was statistically significant (P 0.05). The curative effect of intervertebral disc herniation is remarkable. It has the characteristics of no trauma, simple and convenient operation, favorable price and so on. It is worth popularizing in clinic.
