小儿尿血中医诊疗指南方法学探讨

【摘要】：目的:在文献研究的基础上,采用德尔菲(Delphi)问卷调查与专家论证相结合的方法,制定出科学规范的《小儿尿血中医诊疗指南》。方法:1.文献研究:文献研究包括古代医籍和现代文献两部分内容,采用人工检索、计算机检索和网络检索相结合的方式,查阅古今有关小儿尿血的中医理论、临床研究等资料,整理汇总有关小儿尿血的病因病机、辨证分型、治法方药等信息,并对筛选出的目标文献进行质量评价。2.专家问卷调查:采用Delphi法进行专家问卷调查。在文献研究基础上,制定问卷调查表,并进行三轮问卷调查,对回收的有效问卷进行统计分析,结合专家意见,初步形成《小儿尿血中医诊疗指南》草稿。3.专家论证会:邀请本专业著名的儿科专家和方法学专家,召开专家论证会。提交《小儿尿血中医诊疗指南》草稿,通过专家论证审议修改,最终形成指南初稿。结果:1.通过相关期刊论文共检索出相关文献842篇,其中治疗性文献647篇,理论研究类195篇(经验介绍类107篇,理论探讨及综述88篇);并对文献进行筛选及质量评价,共筛选出符合指南纳入标准的文献309篇。2.三轮问卷调查分别回收有效问卷31、32、35份,专家积极系数依次为88.6%、91.4%、100%,专家积极性高;并在此基础上编制出《小儿尿血中医诊疗指南》草稿。3.召开专家论证会,对调查问卷和指南草稿中存在分歧的地方,如中医辨证分型、其他疗法的应用等问题进行讨论,回收分析专家意见,形成《小儿尿血中医诊疗指南》初稿。结论:1.课题采用文献研究、专家问卷调查和专家论证相结合的方式,初步制订了《小儿尿血中医诊疗指南》初稿,证明这种方法是制定诊疗指南具有可行性的方法。2.课题组编制出的《小儿尿血中医诊疗指南》初稿,专家认可度较高,值得临床应用与推广。
[Abstract]:Objective: on the basis of literature research, the method of combining Delphi Delphi questionnaire with expert argumentation was used. To formulate a scientific and standardized guide to the diagnosis and treatment of pediatric urine and blood. Methods: 1. Literature research: literature research includes ancient medical books and modern literature, and adopts a combination of manual retrieval, computer retrieval and network retrieval. Referring to ancient and modern Chinese medicine theories and clinical studies on children's urine blood, sorting out and collecting information about the etiology and pathogenesis of children's urine blood, differentiation of symptoms and signs, and prescriptions and medicines for treating children's urine blood. The quality of the selected target literature is evaluated. 2. Expert questionnaire: Delphi method is used to carry out expert questionnaire. Based on the literature research, the questionnaire is made, and three rounds of questionnaires are carried out. Through the statistical analysis of the valid questionnaires collected, and combining with the expert opinions, the draft of the guidelines for the diagnosis and treatment of Children's urine and Blood in TCM was formed preliminarily. 3. The expert demonstration meeting: invite the famous pediatrics experts and methodology experts of their major, An expert demonstration meeting was convened. The draft of the "guidelines for the diagnosis and treatment of pediatric urine and blood in traditional Chinese medicine" was submitted, and the first draft of the guide was finally formed through expert argumentation. The result was: 1.A total of 842 related documents were retrieved through the full text database of Chinese periodicals. Among them, there were 647 articles of therapeutic literature, 195 articles of theoretical research (107 articles of experience introduction, 88 articles of theoretical discussion and review). A total of 309 documents, which met the criteria for inclusion of the guidelines, were selected. Three rounds of questionnaires were collected to collect 31 questionnaires and 321,35 valid questionnaires respectively. The positive coefficient of the experts was 88.60.91.4 and 100 respectively, and the enthusiasm of the experts was high. On this basis, the draft of the guidelines for the diagnosis and treatment of Children's urine Blood in TCM was worked out. 3. An expert demonstration meeting was held to discuss the differences in the questionnaire and the draft guidelines, such as the differentiation of symptoms and signs of TCM, the application of other therapies, and so on. The first draft of the guidelines for the diagnosis and treatment of traditional Chinese Medicine in Children's urine Blood was formed. Conclusion 1. The project has preliminarily drawn up the first draft of the "guidelines for the diagnosis and treatment of traditional Chinese Medicine in Children's urine Blood" by means of literature research, expert questionnaires and expert argumentation. It is proved that this method is a feasible method for the development of diagnostic and therapeutic guidelines. The preliminary draft of the guidelines for the diagnosis and treatment of Children's urine Blood in TCM prepared by the research group is highly recognized by experts and is worthy of clinical application and promotion.
