养阴益气汤治疗气阴两虚型子宫内膜不规则脱落的临床研究

【摘要】：目的:观察养阴益气汤在治疗气阴两虚型子宫内膜不规则脱落上的临床疗效,并初步探讨其作用机理,并期待其在临床治疗本病有所借鉴和指导意义。方法:将所搜集60例诊断为气阴两虚型排卵性功血子宫内膜不规则脱落的患者随机分为治疗组(30例)和对照组(30例),治疗组给予中药治疗,对照组给予中成药治疗。所有患者均给予3个月经周期的治疗,主要观察对比两组患者的临床疗效:对比两组患者治疗前后临床症状及体征、基础体温、孕激素水平及子宫内膜厚度的改变情况。结果:临床综合疗效显示,治疗组中痊愈43.33%,显效30.00%,有效13.33%;对照组中分别为痊愈6.67%,显效33.33%,有效13.33%,治疗组的总有效率86.67%远远高于对照组的53.33%,经检验两组的总有效率存在差异,有统计学意义(P0.05);改善中医证候表现上,治疗组中痊愈43.33%,显效30.00%,有效16.67%;对照组中分别为痊愈6.67%,显效33.33%,有效13.33%,治疗组的总有效率90.00%远远高于对照组的53.33%,察两组总有效率存在差异,且有统计学意义(P0.05);中医证候积分方面,经治疗后,两组证候积分均呈下降表现(P0.05),但治疗组低于对照组,差异具有统计学意义(P0.05)。辅助检查方面,治疗组在缩短高温相至低温相的移行时间,调整血清孕酮水平及子宫内膜厚度方面疗效显著,与对照组相比较差异具有统计学意义(P0.05)。结论:养阴益气汤治疗气阴两虚型排卵性功血子宫内膜不规则脱落具有较好的临床疗效,可明显改善患者月经经期、经量、经色、经质及伴随症状、基础体温、孕激素水平及子宫内膜厚度。体现了补肾健脾,养阴益气,固冲宁血的治疗方法在治疗本病上有重要临床意义。
[Abstract]:Objective: to observe the clinical effect of Yangyin Yiqi decoction on irregular exfoliation of endometrium in the type of deficiency of Qi and Yin, and to explore its mechanism. Methods: 60 patients diagnosed as deficiency of qi and yin were randomly divided into treatment group (n = 30) and control group (n = 30). The treatment group was treated with traditional Chinese medicine. The control group was treated with Chinese patent medicine. All patients were treated with three menstrual cycles. The clinical effects of the two groups were observed and compared: the clinical symptoms and signs before and after treatment, the basic body temperature, the clinical symptoms and signs of the patients in the two groups were compared before and after treatment. Conclusion: Yangyin Yiqi decoction has better clinical curative effect on irregular abscission of endometrium of ovulatory type of asthenia of Qi and Yin, and can obviously improve menstrual period, quantity and color of menstruation. Meridian and its concomitant symptoms, basic body temperature, progesterone level and endometrial thickness. The treatment of tonifying kidney and spleen, nourishing yin and invigorating qi, and nourishing Yin and tonifying blood have important clinical significance in treating this disease.
