针刺腹部腧穴为主治疗神经根型颈椎病的临床研究

【摘要】：目的：本课题通过观察针刺腹部腧穴为主治疗神经根型颈椎病的临床疗效,旨在为临床治疗该病寻求更优化的针灸诊治思路与方法。方法：将符合标准且自愿参加本次试验的65例神经根型颈椎病患者(剔除5例)为研究对象,随机分为治疗组与对照组,每组各30例。治疗组以腹部腧穴为主的治疗处方(上脘、中脘、建里、石关、天枢、气海、足三里、内关)治疗；对照组以常规处方(大椎、天柱、颈椎夹脊、后溪、曲池、合谷、外关)治疗。治疗手段均选用毫针针刺法。并以田中晋久20分法量化表、颈椎病临床评价量表作为疗效观测指标,进行两组组间和组内治疗前、治疗后田中晋久20分法量表评分的评分,以及治疗后两组的颈椎病临床评价量表的疗效等级划分的对照研究。结果：研究显示,治疗前两组性别、年龄、病程方面无明显差异(P0.05),两者具有可比性。组间比较中,治疗前两组田中晋久量表总分,经统计P=0.869(P0.05),无统计学意义,可以进行比较。治疗后两组的田中晋久20分法量表总分,经统计P=0.035(P0.05),两者有统计学意义,说明两组治疗后的总分有差别,疗效有差异,可进行比较。就两组治疗后的颈椎病临床评价量表疗效等级的比较,P=0.029(P0.05)两者比较具有统计学意义,治疗组有效率为86.67%,对照组有效率为76.67%,故治疗组疗效高于对照组。组内比较中,治疗组和对照组治疗前后总分统计比较,P=0.00,具有统计学意义,说明两组的治疗方法对神经根型颈椎病都是有效的。结论：两种针刺方法均能改善根型颈椎病的临床症状及一些特定体征,但治疗组有明显的疗效优势。
[Abstract]:Objective: to observe the clinical effect of acupuncture on cervical spondylosis of nerve root type by acupuncture of abdominal acupoints. Methods: 65 patients with cervical spondylopathy of nerve root type (5 cases) who met the criteria and volunteered to participate in the trial were selected as the study objects. The treatment group was randomly divided into treatment group (n = 30) and control group (n = 30). The treatment group was treated with abdominal acupoints (Shangwan, Zhongwan, Jianli, Shiguan, Tianshu, Qihai, Zusanli, Neiguan), while the control group was treated with routine prescriptions (Dazhui, Tianzhu, Tianzhu). Cervical spine clamp, Houxi, Quchi, Hegu, Waiguan) treatment. All the treatment methods were treated by the method of needle acupuncture. The quantitative table of Tianzhong Jinjiu 20's method and the clinical evaluation scale of cervical spondylopathy were used as the therapeutic observation indexes before the treatment between the two groups and within the group. A comparative study on the scores of 20 points of the Tianzhong Jinjiu 20 method after treatment and the clinical evaluation scale of cervical spondylopathy in the two groups after treatment. Results: the study showed that the two groups had sex, age, and age before treatment. There was no significant difference in the course of the disease between the two groups. The total score of Tianzhong Jinjiu scale before treatment in the two groups was not statistically significant, but there was no statistical significance between the two groups. After treatment, the total score of the Tianzhong Jinjiou-20 method scale in the two groups could be compared. There were significant differences in the total score and curative effect between the two groups after treatment. The comparison of the clinical evaluation scale for cervical spondylosis between the two groups was statistically significant. The effective rate was 86.67 in the treatment group and 76.67 in the control group, so the curative effect in the treatment group was higher than that in the control group. Conclusion: the two acupuncture methods can improve the clinical symptoms and some specific signs of cervical spondylopathy of root type, but the treatment group has obvious curative effect advantage.
