益肾泄浊汤治疗肾癌术后合并慢性肾脏病（2-3期）临床观察

【摘要】：目的:通过观察应用益肾泄浊汤治疗肾癌术后合并慢性肾脏病(CKD2-3期)脾肾两虚、浊毒内蕴证患者的治疗前后症状,其肾功、癌胚抗原和D-二聚体水平的变化,探讨其临床疗效。方法:收集符合筛选标准的患者30例,全部患者均来自黑龙江中医药大学附属第一医院肾病科门诊。患者入组后应用益肾泄浊汤治疗,4周为一疗程,共6个疗程。观察对比患者治疗前后症状,肌酐、尿素氮、内生肌酐清除率、CEA和D-二聚体的变化。结果:1.治疗6个月后临床症状明显缓解,P0.01,有显著差异,具有统计学意义。2.Scr、BUN、Ccr数据均符合正态性,治疗中与治疗结束后两组Scr、BUN较治疗前均降低(P0.05),Ccr升高(P0.05),差异具有统计学意义。3.CEA和D-二聚体数据均符合正态性,治疗6个疗程后明显低于治疗前(P0.05),差异具有统计学意义。结论:1.应用益肾泄浊汤可有效改善肾癌术后合并慢性肾脏病(2-3期)脾肾两虚、浊毒内蕴证患者的症状。2.应用益肾泄浊汤在一定程度上可改善患者的肾功、CEA和D-二聚体。
[Abstract]:Objective: to observe the changes of renal function, carcinoembryonic antigen and D-dimer in patients with deficiency of spleen and kidney and accumulation of turbid toxin after treatment with Yishen Xiezhuo decoction (Yishen Xiezhuo decoction). Methods: 30 patients who met the screening criteria were collected, all of whom were from the Department of Renal Department of the first affiliated Hospital of Heilongjiang University of traditional Chinese Medicine. The patients were treated with Yishen Xiezhuo decoction for 4 weeks as a course of treatment. The changes of symptoms, creatinine, urea nitrogen, endogenous creatinine clearance rate CEA and D-dimer were observed and compared before and after treatment for 6 courses. The results showed that Scr-BUNC CCR data were in accordance with normal distribution, and Scr-bun levels in both groups were significantly lower than those before treatment. The difference was statistically significant. 3. CEA and D- dimer data were consistent with normal distribution. After 6 courses of treatment, it was significantly lower than that before treatment (P 0.05), and the difference was statistically significant. Conclusion: 1. The deficiency of spleen and kidney can be effectively improved by applying Yishen Xiezhuo decoction after operation of renal carcinoma complicated with chronic kidney disease at 2-3 stage. Application of Yishen Xiezhuo decoction can improve the renal function of patients with CEA and D-dimer to a certain extent.
