益气化瘀解毒方对慢性萎缩性胃炎癌前病变细胞周期蛋白E表达和肿瘤标记物的影响

【摘要】：目的:探讨益气化瘀解毒方对慢性萎缩性胃炎癌前病变的疗效及对细胞周期蛋白E表达和肿瘤标记物的影响。方法:选择2015年1月—2016年8月在该院各分院门诊及住院的60例确诊为脾虚湿瘀型萎缩性胃炎伴肠上皮或不典型增生患者为研究对象,按照随机数字表法分为对照组(30例)和观察组(30例),对照组给以胃复春治疗,观察组予口服益气化瘀解毒方,24周后观察临床疗效、中医证候评分、胃镜及病理,免疫组化法检测细胞周期蛋白E(Cyclin E)、E-钙黏着蛋白(E-cadherin)和基质金属蛋白酶-9(MMP-9)蛋白表达,酶联免疫方法检测血清胃蛋白酶(PGⅠ和PGⅡ)。结果:治疗6个疗程后,观察组总有效率93.33%高于对照组的66.67%(χ2=6.667,P=0.010);观察组的胃痛、纳差、乏力、嗳气、便溏中医证候积分均低于对照组(P0.05);胃镜下及病理组织学检测观察组胃黏膜萎缩、肠化及异型增生、感染和慢性炎症、糜烂、胆汁反流均优于对照组(P0.05);观察组患者的胃组织Cyclin E和MMP-9蛋白表达低于对照组,而E-cadherin蛋白表达高于对照组(P0.05);观察组患者的PGⅠ和PGⅡ含量高于对照组(P0.05)。结论:益气化瘀解毒方对慢性萎缩性胃炎癌前病变具有较好的临床疗效,可降低中医证候积分,降低细胞周期蛋白E表达,提高血清胃蛋白酶水平,改善患者病情进展。
[Abstract]:Objective: to investigate the effect of Yiqi Huayu jiedu recipe on precancerous lesions of chronic atrophic gastritis and on the expression of cyclin E and tumor markers.Methods: from January 2015 to August 2016, 60 patients with atrophic gastritis with intestinal epithelium or atypical hyperplasia of spleen deficiency and dampness stasis were selected.According to the random number table method, 30 cases were divided into control group (n = 30) and observation group (n = 30).
