李文杰教授从心肾论治老年心力衰竭经验总结

【摘要】：心力衰竭是各种心脏疾病发展到严重阶段的一种复杂的临床症状群,其以患病率高、再入院率高、致残率高、病死率高、生活质量差、预后差的特点而日益受到广泛重视,现已成为广大医学工作者新的研究热点。随着人口老龄化进程的加速及心血管疾病诊疗技术的提高,老年人已成为心力衰竭患者的主体。但由于老年患者机体的特殊性,使常规的治疗受到一定限制,棘手问题较多,反复入院,加重了社会和家庭的负担,故治疗老年心力衰竭已成为当即任务。中医学认识心力衰竭源远流长,经过长期的临床实践已形成自己独特的理论体系,积累了丰富的临床经验和行之有效的治疗方法,避免了西药带来的不良反应,提高了患者的生活质量,对心力衰竭的防治有一定优势。吾师李文杰教授从事中医临床工作30余年,尤其擅长治疗心力衰竭、冠心病、高血压等心血管疾病。吾师对中医基础理论深入理解的基础上,依据老年患者病理生理特点及多年的临床经验,其提出老年患者多心肾阳虚,治疗老年心力衰竭应多从“心肾”论治,采用益气温阳、活血利水之法,临床应用经方真武汤加味治疗,疗效显著。
[Abstract]:Heart failure is a kind of complex clinical symptom group which develops to the serious stage of various heart diseases. It is paid more and more attention for its high prevalence rate, high readmission rate, high disability rate, high fatality rate, poor quality of life and poor prognosis.It has become a new research hotspot for medical workers.With the acceleration of population aging and the improvement of cardiovascular disease diagnosis and treatment, the elderly have become the main body of heart failure patients.However, because of the particularity of the elderly patients, the routine treatment is limited, the thorny problems are many, and the burden of society and family is aggravated by repeated hospitalization, so the treatment of senile heart failure has become an immediate task.Traditional Chinese medicine has a long history of recognizing heart failure. After a long period of clinical practice, it has formed its own unique theoretical system, accumulated rich clinical experience and effective treatment methods, and avoided adverse reactions brought by western medicine.Improve the quality of life of patients, the prevention and treatment of heart failure has a certain advantage.Professor Li Wenjie has been working in TCM for more than 30 years, especially in the treatment of heart failure, coronary heart disease, hypertension and other cardiovascular diseases.On the basis of our deep understanding of the basic theory of traditional Chinese medicine, according to the pathophysiological characteristics of the elderly patients and the clinical experience for many years, we put forward that the elderly patients with multi-heart and kidney yang deficiency should be treated from the "heart and kidney", and the warming of air temperature and yang should be used to treat heart failure in the elderly.The method of promoting blood circulation and promoting water, clinical application of Jingfang Zhenwu decoction treatment, the curative effect is remarkable.
