芪灯振咽汤结合针刺治疗中风后吞咽障碍的临床研究

【摘要】：目的:进一步评价芪灯振咽汤结合针灸治疗中风后吞咽功能障碍气虚血瘀证较单纯针灸治疗是否具有疗效优势,并观察其安全性。方法:依据纳入标准,选取中风后吞咽障碍(气虚血瘀型)患者72例,将患者随机分为对照组和治疗组,治疗组36例,对照组36例。试验组与对照组均给予针刺治疗及常规内科基础治疗,试验组增加口服芪灯振咽汤配合治疗,疗程2周。观察对照组与治疗组患者治疗前后的主要症状、次要症状、洼田饮水试验及相关实验室检查指标的改变,对两组疗法的有效性及安全性进行评价结果:1.治疗结束后,治疗组患者与对照组患者治疗前后洼田饮水试验积分、中医证侯积分分别进行组内比较,结果均有显著的统计学意义(P0.01)。说明两组均有较好的治疗效果。2.治疗前将治疗组患者与对照组患者的洼田饮水试验积分、中医证候积分进行组间比较,结果无统计学意义(P0.05),说明治疗前无明显差异。将两组患者治疗后的洼田饮水试验积分、中医证候积分分别进行组间比较,两个结果均具有统计学差异(P0.05),治疗组患者的洼田饮水试验积分、中医证候积分在治疗后均低于对照组。3.洼田饮水试验疗效:治疗组总有效率80.6%,治愈率为16.13%;对照组总有效率66.7%,治愈率12.12%。4.中医证候疗效:治疗组总有效率93.5%,治愈率为3.2%;对照组总有效率74.19%,治愈率3.0%。治疗前后疗效两组间比较具有统计学意义(P0.05)。5.治疗结束后,两组患者安全性指标均处于正常范围,无明显的皮疹、过敏、休克等不良反应。结论:芪灯振咽汤结合针刺常规治疗效果优于针刺常规治疗,并且使用安全,未发现明显不良反应及毒副作用。
