针刺五脏俞配补母泻子法治疗围绝经期抑郁症的临床观察

【摘要】：目的:通过对比观察五脏俞配补母泻子法与常规针刺治疗肾虚肝郁型围绝经期抑郁症的临床疗效,为临床治疗围绝经期抑郁症提供新的治疗方法。方法:将60例符合标准的肾虚肝郁型围绝经期抑郁症患者随机分成治疗组和对照组,其中治疗组采用五脏俞配补母泻子法治疗,对照组采用常规针刺治疗,两组均每次治疗30min,每日1次,连续治疗6天休息1天为一个疗程,共治疗4个疗程。通过计算汉密顿(HAMD)抑郁量表的减分率评定临床疗效,同时使用中医证候积分表的改善率和库珀曼(Kupperman)评分表(改良版)评判躯体症状的改善情况,最后进行统计学处理,评定五脏俞配补母泻子法治疗肾虚肝郁型围绝经期抑郁症的临床疗效。结果:经4个疗程后治疗组与对照组进行对比得出结论如下:1.两组治疗后总有效率分别为90.0%、80.0%,无显著性差异(P0.05),但治疗组的愈显率高于对照组,说明两组治疗方法均能够改善肾虚肝郁型围绝经期抑郁症的临床症状,且治疗组效果优于对照组。2.治疗后两组HAMD抑郁量表总分及Kupperman评分均有下降,与治疗前比较差异有统计学意义(P0.05);治疗后HAMD抑郁量表总分及Kupperman评分组间比较,有统计学意义,表明治疗组在改善肾虚肝郁型围绝经期抑郁症的症状方面优于对照组。3.两组治疗前中医证候积分的比较差异无统计学意义(P0.05);治疗组治疗后中医证候积分较治疗前降低,治疗前后比较有显著性差异(P0.05);治疗组和对照组治疗前后差值比较,具有显著性差异,说明治疗组在改善肾虚肝郁型围绝经期抑郁症的躯体症状方面要优于对照组。结论:五脏俞配补母泻子法与普通针刺均能明显改善肾虚肝郁型围绝经期抑郁症患者的抑郁程度、躯体症状,五脏俞配补母泻子法对肾虚肝郁型围绝经期抑郁症的临床疗效优于普通针刺疗法。
[Abstract]:Objective: the clinical curative effect of the treatment of kidney deficiency and liver stagnation of perimenopausal depression reinforcing mother diarrhea and routine acupuncture method by comparison with five Zangshu, for the clinical treatment of perimenopausal depression and provide new treatment. Reduce medicine syndrome score than before treatment, with significant difference before and after treatment (P0.05); the treatment group and control group before and after treatment the difference, with significant differences, indicating somatic symptoms than the control group, the treatment group in improving kidney deficiency and liver stagnation of perimenopausal depression. Conclusion: the degree of depression, five Zangshu filled mother and son method and ordinary acupuncture can significantly improve kidney deficiency and liver stagnation of perimenopausal depression with somatic symptoms, five Zangshu filled mother purging method of kidney deficiency and liver stagnation of perimenopausal depression clinical curative effect is better than ordinary acupuncture therapy.
