拔罐结合针刺改善慢性湿疹瘙痒症状临床疗效观察

【摘要】：目的:本课题通过拔罐结合针刺与常规针刺治疗慢性湿疹改善瘙痒症状临床疗效的对比,旨在优化临床治疗思路,为针刺配合拔罐疗法治疗慢性湿疹的临床应用推广提供证据支持和理论依据。方法:采用简单随机化的方法将符合纳入标准的60例慢性湿疹患者按照1:1分为治疗组30例,对照组30例。治疗组采用拔罐(肩耦穴、血海穴、日月期门穴、神阙穴以及环跳穴)+毫针针刺(定神针、丘墟、神门、曲池、足三里、三阴交、阴陵泉、皮损局部)+温针灸气海穴;对照组采用单纯针刺治疗(定神针、丘墟、神门、曲池、足三里、三阴交、阴陵泉、皮损局部)+温针灸气海穴。拔罐,可视罐印颜色的深浅,每周治疗1-2次;针刺每周治疗2次。4周为1疗程,治疗2个疗程。每个疗程结束后,对慢性湿疹瘙痒积分以及皮肤病生活质量量表(DLQI)积分与治疗前积分进行比较分析。结果:60例受试者中,依据剔除、脱落标准剔除3例。治疗组实为29例,对照组实为28例。通过瘙痒积分计算出疗效率,治疗1个疗程后,治疗组的总有效率为79.31%,对照组的总有效率为75.00%,经秩和检验,P0.05,差异有统计学意义,说明短期的治疗效果治疗组较对照组明显;治疗2个疗程后,治疗组的总有效率为96.55%,对照组的总有效率为89.29%,经秩和检验,P0.05,差异有统计学意义,说明长期的治疗效果治疗组较对照组明显。治疗组的DLQI积分差为9.34±2.43,对照组DLQI积分差值为6.96±2.01。经秩和检验,P0.05,差异有统计学意义。说明在改善生活方面,治疗组的效果比对照组效果明显。结论:针灸治疗慢性湿疹总体疗效明确,结合拔罐后治疗效果更显著。研究结果表明,拔罐结合针刺治疗慢性湿疹在瘙痒症状减轻方面效果突出,在生活质量改善方面的疗效亦明显。这种方法可以明显减少服用药物的副作用,值得临床推广。
[Abstract]:Objective: to compare the clinical effects of cupping combined with acupuncture and conventional acupuncture in the treatment of chronic eczema. To provide evidence and theoretical basis for clinical application of acupuncture combined with cupping therapy in the treatment of chronic eczema. Methods: 60 patients with chronic eczema were randomly divided into treatment group (30 cases) and control group (30 cases) according to 1:1. The treatment group was treated with cupping (shoulder coupling point, Xuehai point, sun and moon period gate point, Shenque point and Huanqiu point) and acupuncture at warm air sea point (Ding Shen acupuncture, Qiu Hui, Shenmen, qu Chi, Zusanli, Sanyinjiao, Yin Ling Quan, local lesions). The control group was treated with pure acupuncture (Ding Shen acupuncture, Qiu Hui, Shenmen, qu Chi, Zusanli, Sanyinjiao, Yin Lingquan, local lesions). Cupping, visible ink color, 1-2 times a week, acupuncture treatment twice a week for 4 weeks as a course of treatment, 2 courses of treatment. After each course of treatment, the scores of chronic eczema pruritus and dermatological quality of life scale (DLQI) were compared with those before treatment. Results among 60 subjects, 3 cases were excluded according to elimination and 3 cases by shedding criterion. There were 29 cases in the treatment group and 28 cases in the control group. After one course of treatment, the total effective rate of the treatment group was 79.31 and that of the control group was 75.000.The difference was statistically significant by rank sum test (P 0.05), indicating that the short-term therapeutic effect in the treatment group was more obvious than that in the control group. After two courses of treatment, the total effective rate of the treatment group was 96.555.The total effective rate of the control group was 89.29. The difference was statistically significant by rank sum test (P0.05), indicating that the long-term therapeutic effect in the treatment group was more obvious than that in the control group. The difference of DLQI score was 9.34 卤2.43 in the treatment group and 6.96 卤2.01in the control group. The difference was statistically significant by rank sum test (P 0.05). It showed that the effect of the treatment group was better than that of the control group. Conclusion: the total curative effect of acupuncture and moxibustion on chronic eczema is clear, especially after cupping. The results showed that cupping combined with acupuncture was effective in relieving pruritus and improving quality of life. This method can significantly reduce the side effects of taking drugs, and is worthy of clinical promotion.
