【摘要】：背景:乳腺癌是现代女性高发的恶性肿瘤,随着科技进步时代发展生活节奏的加快,都市女性面临的社会工作家庭诸多问题所承受的的压力也比以前变得复杂多样,近年乳腺癌发病率也有了明显的上升趋势,其发病率及死亡率已居我国女性恶性肿瘤之首[1],其中在我国北京、上海、深圳等沿海发达地区的发病率明显高于内陆偏远贫困地区。化疗作为治疗肿瘤疾病的重要手段之一,虽然在控制局部复发和全身转移方面效果显著,然由于化疗药物的低选择性,往往对正常细胞造成误杀,抑制了机体的免疫功能,常表现为恶心呕吐、食欲不振、疲乏倦怠、烦躁失眠、毛发脱落、脏器功能损伤、腹痛、腹泻或便秘等毒副反应,常常使术后本已虚弱的患者心力交瘁、受尽折磨,严重的甚至被迫放弃治疗中断化疗。为了减轻这些毒副反应,使病人顺利完成全部治疗,西医采取了一系列相应措施以期缓解化疗所产生的临床症状,然效果皆不尽人意,有的甚至进一步损伤肝肾等脏器,带来新的毒副反应。寻找行之有效的治疗方法,降低乳腺癌化疗的毒副反应,成为临床关注探索的焦点。为了解决这些问题,近年一些临床医生渐渐开始尝试在乳腺癌化疗过程中配合相应的中药治疗,以缓解化疗药物的毒副作用,并取得了较好的治疗效果。由此,中医药在治疗乳腺癌化疗毒副反应中,以其绿色有效无损害的显著优势逐渐的受到临床医生和患者的青睐,如何使其更广泛的应用于临床成为现代医学探索专研的热点。目的:解建国教授从医四十余载,临证经验丰富,对乳腺癌化疗所引起的一系列副反应的治疗更是有着独到的见解。本文将就解教授治疗乳腺癌化疗副反应的学术特点、用药思路加以介绍阐述,希望可以为临床治疗乳腺癌化疗副作用提供新方法,并为将来中医药在降低肿瘤对化疗药物的耐药性及促进术后恢复、控制肿瘤发展、复发转移等的治疗上提供新思路。研究方法:经过近3年的跟师学习,结合临床典型病案加以剖析体会,整理相关资料,查阅国内外文献,将解师治疗乳腺癌化疗副反应的辨证思路及用药特点加以归纳总结。结果:根据乳腺癌患者化疗所产生副反应的不同程度及特点,采用中药汤剂,灵活加减,辨证论治,有效的缓解了乳腺癌化疗引起的恶心、呕吐、腹泻或便秘、疲乏、失眠等毒副反应,改善食欲不振,激发自身免疫力,明显提高了病人的生活品质,延长了生存期。结论:解师自拟乳腺癌化疗副反应经验方有效缓解了乳腺癌患者化疗所产生毒副反应,减轻了病人痛苦,使其顺利完成全部化疗,在临床中取得了显著效果,其治疗经验值得我们学习研究及传承应用。
[Abstract]:Background: breast cancer is a malignant tumor with a high incidence among modern women. With the rapid pace of development of life in the era of scientific and technological progress, the pressure on urban women's social work and family problems has become more complex and diverse than before. In recent years, the incidence of breast cancer has also increased obviously. The incidence and mortality of breast cancer have been ranked first among the female malignant tumors in China. The incidence of breast cancer in developed coastal areas such as Beijing, Shanghai, Shenzhen and so on is obviously higher than that in remote and poor inland areas. Chemotherapy is one of the important methods in the treatment of tumor diseases. Although it has a remarkable effect in controlling local recurrence and systemic metastasis, however, because of the low selectivity of chemotherapeutic drugs, it often causes manslaughter to normal cells and inhibits the immune function of the body. Often manifested as nausea, vomiting, loss of appetite, fatigue, restlessness, insomnia, hair loss, organ function damage, abdominal pain, diarrhea or constipation and other toxic side effects, often make the patient who is already weak after the operation, exhausted and tormented, Severe even forced to abandon treatment to interrupt chemotherapy. In order to alleviate these side effects and make the patients complete all the treatment successfully, Western medicine took a series of corresponding measures to alleviate the clinical symptoms produced by chemotherapy. However, the effect was not satisfactory, and some even further damaged the organs such as liver and kidney. Bring new side effects. It has become the focus of clinical exploration to find effective treatment methods to reduce the side effects of breast cancer chemotherapy. In order to solve these problems, in recent years, some clinicians began to try to cooperate with traditional Chinese medicine in the process of breast cancer chemotherapy, in order to alleviate the toxic and side effects of chemotherapy drugs, and achieved a good therapeutic effect. As a result, traditional Chinese medicine in the treatment of breast cancer chemotherapy side effects, because of its significant advantage of green effectiveness and no damage gradually favored by clinicians and patients, how to make its wider application in clinical practice has become a hot spot of modern medical exploration. Objective: professor Xie Jianguo has been in medicine for more than 40 years and has rich clinical experience. He has a unique opinion on the treatment of a series of side effects caused by chemotherapy of breast cancer. This article will introduce Professor Jie's academic characteristics of side effects of chemotherapy for breast cancer and the thinking of drug use in order to provide a new method for the treatment of side effects of chemotherapy in breast cancer. It also provides new ideas for the treatment of tumor resistance to chemotherapeutic drugs, promoting postoperative recovery, controlling tumor development, recurrence and metastasis in the future. Methods: after nearly 3 years' study, combined with the clinical typical medical records, the author analyzed and analyzed the relevant materials, consulted the domestic and foreign literature, and summed up the syndrome differentiation thinking and the characteristics of drug use in the treatment of the side effects of chemotherapy in breast cancer. Results: according to the different degree and characteristics of the side effects caused by chemotherapy in breast cancer patients, traditional Chinese medicine decoction, flexible addition and subtraction, differentiation of symptoms and signs were used to effectively relieve nausea, vomiting, diarrhea or constipation caused by chemotherapy of breast cancer, and fatigue. The side effects, such as insomnia, improve anorexia, stimulate immunity, improve the patient's quality of life and prolong the life span. Conclusion: the experience prescription of self-made chemotherapy side effects of breast cancer can effectively alleviate the side effects of chemotherapy in patients with breast cancer, alleviate the pain of patients, make it complete all chemotherapy smoothly, and achieve remarkable results in clinical practice. Its therapeutic experience is worth studying and inheriting application.
