【摘要】：目的:应用Delphi法制定出科学规范的“小儿细菌性痢疾中医诊疗指南”。方法:结合文献研究的结果、采用专家问卷调查(Delphi法)、专家论证会及行业专家征求意见相结合的方法制订“小儿细菌性痢疾中医诊疗指南”。专家问卷调查:在文献研究成果的基础上,制定第一轮专家调查问卷,采用电子邮件的方式,向全国范围内遴选的40位专家发送问卷。对回收的第一轮调查问卷进行统计分析,形成第二轮专家调查问卷。将第二轮问卷发送给回复第一轮问卷的专家及再次遴选的其他专家共40位(第一轮未回复专家不再发送),再次统计分析第二轮回收的问卷,并将前两轮专家所提出的共性问题作为第三轮纳入指标,形成第三轮调查问卷。将第三轮问卷发送给回复第二轮问卷的专家及再次遴选的其他专家共40位(第二轮未回复专家不再发送),再次统计分析第三轮回收的问卷,所得问卷即“小儿细菌性痢疾中医诊疗指南”的草稿。专家论证会:邀请国内著名中医儿科专家,召开专家论证会,对“小儿细菌性痢疾中医诊疗指南”的草稿进行讨论、论证。最后根据专家讨论共识,结合课题负责人修改意见,形成“小儿细菌性痢疾中医诊疗指南”的初稿。行业专家征求意见:将“小儿细菌性痢疾中医诊疗指南”初稿报送专家指导组论证,根据专家指导组意见修改完善并征求同行意见,形成指南评价稿。结果:在文献研究的基础上,编制专家问卷调查表,共进行了三轮专家咨询,专家积极系数分别为77.5%、92.5%、92.5%,在第三轮问卷中专家意见已基本趋于一致,因此结束专家咨询,并通过专家会议的论证,形成符合临床实际的科学规范的“小儿细菌性痢疾中医诊疗指南”初稿。并将其报送专家指导组论证,根据专家指导组意见修改完善并征求同行意见,形成指南评价稿。结论:应用Delphi法进行小儿细菌性痢疾中医诊断、辨证、治疗、预防和调护的规范化研究,并根据专家所提供的意见和建议,制订了“小儿细菌性痢疾中医诊疗指南”,同时证明Delphi法作为一种充分发挥专家智慧、知识和经验的方法,具有一定的科学性和可操作性,是中医诊疗规范化研究中具有客观性、传统性及可操作性的重要方法学。
[Abstract]:Objective: to establish a scientific standard guide for the diagnosis and treatment of bacillary dysentery in children by Delphi method. Methods: according to the results of literature research, the method of Delphi method was used to make the guidance of traditional Chinese medicine diagnosis and treatment of bacillary dysentery in children. Expert questionnaire: based on the results of literature research, the first round of expert questionnaire was made, and the questionnaire was sent to 40 experts selected in the whole country by email. The second round of expert questionnaire was formed by statistical analysis of the first round of questionnaire. The second round of the questionnaire was sent to the experts who responded to the first round of the questionnaire and the other experts who were re-selected. The common problems raised by the experts in the first two rounds are taken as the index of the third round, and the third round questionnaire is formed. The third round of the questionnaire was sent to 40 experts who responded to the second round of the questionnaire and other experts who were re-selected. The questionnaire is the draft of the Guide to traditional Chinese Medicine diagnosis and treatment of Pediatric bacillary dysentery. Expert demonstration meeting: invite famous Chinese medicine paediatrics experts to hold expert demonstration meeting to discuss and demonstrate the draft of the guide of traditional Chinese medicine diagnosis and treatment of pediatric bacillary dysentery. Finally, according to the consensus of the expert discussion and the revised opinion of the person in charge of the project, the first draft of the guidelines for the diagnosis and treatment of Pediatric bacillary dysentery was formed. The first draft of the guide for the diagnosis and treatment of pediatric bacillary dysentery was submitted to the expert guidance group for argumentation, which was revised and perfected according to the opinion of the expert guidance group and the peer opinion was solicited to form the evaluation draft of the guide. Results: on the basis of literature study, the questionnaire of experts was compiled and three rounds of expert consultation were conducted. The positive coefficient of experts was 77.5 and 92.5, respectively. In the third round of questionnaires, the opinions of experts had basically converged, so the expert consultation was concluded. Through the demonstration of the expert meeting, the first draft of the guidelines for the diagnosis and treatment of Pediatric bacillary dysentery in accordance with the scientific norms of clinical practice was formed. And submit it to the expert guidance group for argumentation, revise and perfect according to the expert guidance group's opinion and solicit the peer opinion, and form the evaluation draft of the guide. Conclusion: using Delphi method to study the standardization of TCM diagnosis, differentiation, treatment, prevention and nursing care of children bacillary dysentery, and according to the opinions and suggestions provided by experts, we have formulated the guidelines of traditional Chinese medicine diagnosis and treatment of pediatric bacillary dysentery. At the same time, it is proved that Delphi method, as a method of giving full play to expert wisdom, knowledge and experience, is scientific and operable, and it is an important methodology with objectivity, tradition and maneuverability in the standardization research of TCM diagnosis and treatment.
