【摘要】：目的:探索导师唐农教授运用其学术理论思想治疗老年恶性肿瘤的经验,梳理唐农教授学术理论思想渊薮以及主要学术理论,研究“本土学说”在老年恶性肿瘤治疗中的重要意义,总结唐农教授对老年恶性肿瘤病因病机的认识,以及治法治则的规律。方法:其一,运用文献检索,综述古今中医对老年恶性肿瘤的认识,以及现代中医药治疗老年恶性肿瘤的研究现状。其二,通过系统研究导师唐农教授的著述,梳理唐农教授学术理论思想渊薮以及相关学术理论;结合钦安卢氏医学著作,重点研究“本土学说”理论,并与相关中土理论思想进行比较。其三,收集整理唐农教授治疗老年恶性肿瘤的案例,联系“本土学说”及相关学术理论思想,总结在唐农教授学术理论思想指导下,运用次第法则治疗老年恶性肿瘤的临床经验。结果:唐农教授的学术理论思想体系以钦安卢氏扶阳医学为基础,以中华文化经典的宇宙生成观为背景,深刻揭示人之生命现象本质。唐农教授的主要学术理论包括“三论”、“七大法则”,其中以“中医阴阳本体结构理论”和“中医本土学说理论”最为核心,揭示人体阴阳结构为内阳外阴的本体结构,人之生命整体的本质属性在五行中属土,并正确理解扶正祛邪等中医治则的根本含义。认识老年恶性肿瘤的病因病机为阴阳本体结构的破坏、人体本土属性的衰败和后天中土系统的失衡,治疗应设法恢复人体内阳外阴的本体结构,修复人体的本土属性,回归后天中土系统的平衡状态。具体治疗时,提出次第治疗的三大法则,分别为“温通中焦,恢复中气”、“温化内寒,疏通郁热”和“培土填精,固本培元”,根据次第治疗原则灵活遣方用药,强调临证始终不离六经辨证,注重把握阳气与中气在治疗中的关键地位。结论:唐农教授的学术理论思想契合中医经典著述,还原经典本义,有助于理解中医理论的本质内涵,理明则用达,能有效指导临床实践,具有自洽性,是对中医学理论及临证的发展。中医药在治疗老年恶性肿瘤方面具有独特优势,运用唐农教授学术理论思想指导老年恶性肿瘤的治疗,有助于厘清老年恶性肿瘤的病因病机以及治疗原则,尤其在“本土学说”的指导下,形成了理法方药的完备体系,为中医临床系统治疗老年恶性肿瘤提供了新思路新方法。
