针刺结合中药药浴治疗脑卒中后上肢痉挛的临床疗效观察

【摘要】：目的：探讨针刺结合中药药浴治疗脑卒中后上肢痉挛的临床疗效。方法：将纳入的脑卒中后上肢痉挛的60例患者随机分为治疗组和对照组,每组患者各30例。对照组选取针刺治疗,治疗组在对照组基础上进行中药药浴法治疗,每日治疗1次,每6日为一个疗程,每个疗程间休息1天,连续治疗4个疗程。观察两组患者在治疗前后Fugl-Meyer上肢功能评分(FMA)、改良Ashworth量表(MAS)、修订的Barthel指数(MBI)及表面肌电均方根值(RMS)的疗效变化。结果：1.两组患者治疗前后Fugl-Meyer评分比较：治疗前两组评分比较无显著差异(P0.05),治疗后两组评分组内比较有显著差异(P0.05),组间比较有显著性差异(P0.01)。2.两组患者治疗前后改良Ashworth量表评定比较：治疗前两组患者痉挛程度比较无显著差异(P0.05),治疗后组间比较有显著性差异(P0.05)。3.两组患者治疗前后MBI评分比较：治疗前两组评分比较无显著性差异(P0.05),治疗后两组评分组内比较均有显著性差异(P0.05),组间比较有显著性差异(P0.05)。4.两组患者治疗前RMS积分比较：治疗前两组评分比较无显著性差异(P0.05),治疗后两组评分组内比较均有显著性差异(P0.05),组间比较有显著性差异(P0.01)。结论：1.针刺结合中药药浴能有效改善脑卒中后上肢痉挛患者的上肢运动功能,疗效优于单纯针刺组。2.针刺结合中药药浴能有效改善脑卒中后上肢痉挛患者的日常生活活动能力,疗效优于单纯针刺组。3.针刺结合中药药浴能有效降低脑卒中后上肢痉挛患者肢体的肌张力,减轻功能障碍。4.针刺结合中药药浴能显著降低脑卒中后上肢痉挛患者的RMS积分。
[Abstract]:Objective: to investigate the clinical effect of acupuncture combined with traditional Chinese medicine in the treatment of upper limb spasm after stroke. Methods: 60 patients with upper limb spasm after stroke were randomly divided into treatment group and control group with 30 cases in each group. The control group was treated with acupuncture, the treatment group was treated with traditional Chinese medicine medicine bath method on the basis of the control group, once a day, every 6 days as a course of treatment, one day of rest between each course of treatment, 4 consecutive courses of treatment. The curative effects of Fugl-Meyer upper limb function score, modified Ashworth scale, revised Barthel index and RMS were observed before and after treatment. The result is 1: 1. Comparison of Fugl-Meyer scores before and after treatment: there was no significant difference between the two groups before and after treatment (P 0.05), but there was a significant difference between the two groups after treatment (P 0.05), and there was a significant difference between the two groups (P 0.01 路2). Comparison of modified Ashworth scale before and after treatment: there was no significant difference in spasticity between the two groups before and after treatment, but there was significant difference between the two groups after treatment. Comparison of MBI score before and after treatment: there was no significant difference between the two groups before and after treatment (P 0.05), but there was significant difference between the two groups after treatment (P 0.05), and there was significant difference between the two groups (P 0.05 路4). Comparison of RMS scores before treatment: there was no significant difference between the two groups before treatment (P 0.05), but there was significant difference between the two groups after treatment (P 0.05), and there was significant difference between the two groups (P 0.01). Conclusion 1. Acupuncture combined with traditional Chinese medicine can effectively improve the upper limb motor function of patients with spasm of upper limb after stroke, and the curative effect is better than that of simple acupuncture group. 2. Acupuncture combined with traditional Chinese medicine can effectively improve the activities of daily life of patients with spasm of upper limbs after stroke, and the curative effect is better than that of acupuncture group. 3. Acupuncture combined with traditional Chinese medicine can effectively reduce the muscle tension of the upper limb spasm patients after stroke and relieve the dysfunction. 4. Acupuncture combined with traditional Chinese medicine can significantly reduce the RMS score of patients with upper limb spasm after stroke.
