“改良青龙摆尾”针法治疗膝痹（瘀血闭阻证）临床疗效研究

【摘要】：目的:验证针灸疗法是治疗KOA的有效疗法之一,通过对改良的青龙摆尾针法对于KOA患者疗效的验证,推广改良的青龙摆尾针法,进一步指导KOA的临床治疗。方法:选取2014-2015年就诊于湖北省中医院针灸科的KOA患者40例,随机分为青龙摆尾针法组和普通针刺组,两组取穴均为:血海、犊鼻、内膝眼、膝阳关、太溪、太白、居毼、交信。对照组采用普通针刺法,治疗组居毼、交信两穴采取改良的青龙摆尾针法,余穴采取普通针刺法。分别记录两组治疗前后的VAS疼痛评分、中医症状评分、下肢功能评分,并通过统计学分析,比较两组疗效的优劣。并通过文献整理阐述青龙摆尾针法、膝关节骨性关节炎目前国内外研究进展,对改良的青龙摆尾针法治疗KOA的优越性进一步从理论验证。结果:1.VAS评分:治疗前青龙摆尾针法组与普通针刺组经t检验,t=-0.1089,p=0.91380.05,治疗前后普通针刺组VAS评分经t检验,t=-6.7939,p0.01,具有统计学差异,治疗前后青龙摆尾针法组VAS评分经t检验,t=-8.5099,p0.01,具有统计学差异,青龙摆尾针法组与普通针刺组治疗前后VAS评分差值的绝对值经t检验,t=-2.5501,p=0.01490.05。2.中医症状评分:治疗前青龙摆尾针法组与普通针刺组经t检验,t=0.0662,p=0.94750.05,治疗前后普通针刺组中医症状评分经t检验,t=-6.5194,p0.01,具有统计学差异,治疗前后青龙摆尾针法组中医症状评分经t检验,t=-4.9218,p0.01,具有统计学差异,青龙摆尾针法组与普通针刺组治疗前后中医症状评分差值的绝对值经t检验,t=-3.5324,p=0.00110.01。3.下肢功能评分:治疗前青龙摆尾针法组与普通针刺组经t检验,t=1.4587,p=0.15290.05,治疗前后普通针刺组下肢功能评分经t检验,t=--6.0308,p0.01,具有统计学差异,治疗前后青龙摆尾针法组中医症状评分经t检验,t=12.5626,p0.01,青龙摆尾针法组与普通针刺组治疗前后下肢功能评分差值的绝对值经t检验,t=-4.1406,p=0.00020.01。4.临床疗效:两组疗效经卡方检验,p=0.01650.05,具有可比性。结论:1.改良的青龙摆尾针法对于治疗膝关节骨性关节炎的疗效明显优于普通针刺法。2.总而言之,改良的青龙摆尾针法对于膝关节骨性关节炎患者的疼痛程度、中医症状、下肢功能均具有显著疗效,但目前国内对其的专项研究较少,应进一步加大其研究及临床推广的力度。
[Abstract]:Objective: to verify that acupuncture and moxibustion therapy is one of the effective treatments for KOA. Through the validation of the effect of modified Azure Dragon wobble tail acupuncture on KOA patients, the modified Azure Dragon wobble tail acupuncture method is extended to guide the clinical treatment of KOA. Methods: forty patients with KOA were randomly divided into two groups: Xuehai, calf nose, inner knee eye, knee Yangguan, Taixi, Taibai, Juyi and Jiaoxin, which were randomly divided into two groups: Xuehai, calf nose, inner knee eye, Yangguan of knee, Taibai, Juyi and Jiaoxin. The control group was treated with common acupuncture, the treatment group was treated with modified Azure Dragon tail acupuncture and the remaining acupoints were treated with common acupuncture. VAS pain score, TCM symptom score and lower limb function score were recorded before and after treatment, and the efficacy of the two groups was compared by statistical analysis. The research progress of Azure Dragon wobble tail acupuncture and knee osteoarthritis at home and abroad was expounded through literature review. The superiority of modified Azure Dragon wobble tail acupuncture in the treatment of KOA was further verified theoretically. Results 1. VAS score: before treatment, the VAS score of Azure Dragon wobble tail acupuncture group and common acupuncture group was t test -0.1089% p0.91380.05, and before and after treatment, the VAS score of common acupuncture group was t test -6.7939% p0.01, there was statistical difference between the two groups. Before and after treatment, the VAS score of the Azure Dragon tail swinging acupuncture group was significantly different by t test (t test). The absolute value of VAS score difference between the Azure Dragon tail swinging acupuncture group and the common acupuncture group was 0.01490.05.2 by t test. The scores of TCM symptoms: before treatment, the scores of TCM symptoms in Azure Dragon tail acupuncture group and ordinary acupuncture group were examined by t test and t test respectively. Before and after treatment, the scores of TCM symptoms in common acupuncture group were t test -6.5194 / p0.01, and there was statistical difference between them, and there were significant differences between the two groups before and after treatment. Before and after treatment, the TCM symptom score of Azure Dragon tail swinging acupuncture group was statistically different by t test (t test). The absolute value of TCM symptom score difference before and after treatment in Azure Dragon tail swinging acupuncture group and common acupuncture group was measured by t test (t test)-3.5324g / 0. 00110.01.3. Lower extremity function score: before treatment, the lower extremity function score of Azure Dragon acupuncture group and common acupuncture group were 1.4587 and 6.0308, respectively, which were significantly different before and after treatment, and 0.15290.05 before and after treatment, and there was statistical difference between the two groups before and after treatment, and there was significant difference between the two groups before and after treatment, and the score of lower extremity function was 0.15290.05before and after treatment, there was statistical difference between the two groups. Before and after treatment, the TCM symptom score of Azure Dragon tail swinging acupuncture group was evaluated by t test 12.5626 p0.01, and the absolute value of lower limb function score difference between Azure Dragon tail swinging acupuncture group and common acupuncture group was measured by t test (t test). Clinical efficacy: the two groups were comparable by chi-square test (P 0.01650.05). Conclusion 1. The effect of modified Azure Dragon method on knee osteoarthritis is better than that of common acupuncture. 2. In a word, the modified Azure Dragon method has a significant effect on the pain degree, TCM symptoms and lower limb function of patients with knee osteoarthritis, but there are few special researches on it in China. Its research and clinical popularization should be further strengthened.
