基于推拿教材与文献研究拨法的临床应用

【摘要】：背景：本论文基于教材、文献和临床经验,深入研究拨法的演变、差异和应用;并通过导师于天源教授的临床病例,阐明了拨法的应用。文章主要讨论的治疗手法--拨法,是按摩推拿治疗肌肉、韧带、肌腱相关疾病的重要手法。于天源教授从事推拿治疗多年,并从实践中总结出一套新方法。此方法以经络理论为基础,并着眼于手法的机制和疗效。本论文目的是定义、阐明、评价拨法的演变、疗效和安全性。目的:通过查阅教科书、综述文献和总结导师于天源教授在治疗神经根型颈椎病的临床经验,研究拨法的演变、疗效和应用。方法:文章以研究拨法的演变、应用、疗效和治疗肌肉、肌腱、韧带相关疾病的原理为目的,综述了中国知网的相关文章,分析了过去57年间推拿教材上的拨法,以及总结了使用拨法治疗神经根型颈椎病的病例。在中国知网搜索使用拨法治疗多种疾病及其相关,共得到1991至2017年间的129篇文献。剔除掉缺乏证明及重复的文章,64篇文章有参考意义,而本文引用了 10篇主题相关文章。其余文章因缺乏拨法演变或以其他手法为主而被剔除。本文引用了自1959至2016年,共57年间的推拿教科书,以阐述拨法的演变和历史。并以拨法在神经根型颈椎病中的应用为例,总结了导师于天源教授在国医堂出诊时,拨法在治疗过程中的应用。结果:1.通过近十年拨法的文献比较发现,对拨法疗效的理解已日益深入,论证了近年来拨法的发展进步,并成为推拿治疗疼痛及其相关疾病的主要手法,在治疗例如颈椎病、腰椎间盘突出症等肌肉和神经相关疾病中,取得了显著疗效.2.文章亦观察了拨法如何单独使用和与其他手法,例如揉法;针灸和热敷共同使用,以期在伤科疾病的治疗中取得更好效果。此外,未观察到拨法产生明显副作用,且拨法在疼痛缓解方面有显著效果。结论:近年来,拨法有了显著提高,并成为现代推拿治疗的主要手法。在临床应用中,疗效显著,实用性强。拨法可以作为无副作用的手法,治疗疼痛及相关疾病。因此可得出结论,拨法可作为单一治疗手段,应用于肌肉、肌腱、韧带疼痛。
[Abstract]:Background: based on the teaching materials, literature and clinical experience, this paper deeply studies the evolution, difference and application of the allocation method, and illustrates the application of the method through the clinical case of Professor Yu Tianyuan. This paper mainly discusses the treatment of muscle, ligaments and tendon-related diseases by massage and massage. Professor Yu Tianyuan engaged in massage therapy for many years and summed up a new method from practice. This method is based on meridian theory and focuses on the mechanism and efficacy of manipulation. The purpose of this paper is to define, clarify, and evaluate the evolution, efficacy and safety of allocative methods. Objective: to review the literature and summarize the clinical experience of professor Yu Tianyuan in the treatment of cervical spondylopathy of nerve root type, and to study the evolution, curative effect and application of the method. Methods: in order to study the evolvement, application, curative effect and the principle of treating muscle, tendon and ligament-related diseases, the article summarized the related articles of Chinese knowledge net, and analyzed the methods of manipulation in the past 57 years. The cases of cervical spondylosis of nerve root type were summarized. A total of 129 articles were collected from 1991 to 2017 in China to search for the use of dialing method in the treatment of various diseases and related diseases. Eliminating the lack of proof and repetition of 64 articles is useful, while this paper cites 10 relevant articles on the subject. The rest of the articles were rejected for lack of evolution or other techniques. In this paper, 57 years of massage textbooks from 1959 to 2016 are cited to illustrate the evolution and history of the allocation method. Taking the application of plucking method in cervical spondylopathy of nerve root type as an example, the application of professor Yu Tianyuan, in the course of treatment was summarized. The result is 1: 1. Through the comparison of literature in the past ten years, it is found that the understanding of the curative effect of the method has been deepening day by day, and the development and progress of the method in recent years has been demonstrated, and it has become the main technique for massage treatment of pain and related diseases, such as cervical spondylosis, in the treatment of cervical spondylopathy. In muscle and nerve related diseases such as lumbar disc herniation, significant curative effect was obtained. 2. 2. The article also observed how to use it alone and with other techniques, such as kneading, acupuncture and hot compress, in order to achieve better results in the treatment of injuries. In addition, no obvious side effects were observed, and there was significant effect in pain relief. Conclusion: in recent years, the modus operandi has been improved significantly, and has become the main technique of modern massage therapy. In the clinical application, the curative effect is remarkable, the practicability is strong. It can be used to treat pain and related diseases without side effects. Therefore, it can be concluded that the method can be used as a single treatment for muscle, tendon and ligament pain.
