三法三穴干预坐骨神经损伤大鼠运动功能刺激参数研究

【摘要】:[目的]通过正交试验指导下比较行为学、腓肠肌功能、免疫组织化学相关指标的改变,从行为学下研究推拿手法参数中的力量、时间、频率对周围神经损伤大鼠运动功能恢复的影响。探究推拿促进坐骨神经损伤大鼠运动功能恢复的最佳刺激参数组合。[方法]以坐骨神经损伤为例研究周围神经损伤,以SD大鼠为实验动物,采用坐骨神经加持损伤模型。7天后使用"推拿手法模拟仪"模拟三法(拨法、揉法、点法)推拿手法在三个穴道(殷门、承山、阳陵泉)上进行干预。以力量(A)、时间(B)、频率(C)三种因素,每个因数各有三组不同水平刺激,力量为2N、4N、6N;时间1min、3min、5min;频率30次/min、60次/min、100次/min。用三因素三水平的正交试验设计方案筛选推拿干预促进SNI大鼠运动功能恢复的最佳刺激参数组合。1.从行为学角度,测定斜板实验及坐骨神经功能指数以评价大鼠肌力功能恢复情况。2.从功能角度,测定腓肠肌湿重恢复率评价推拿对肌肉萎缩程度的改善。3.采用免疫组织化学法测定脊髓腹角内NGF的表达情况,探寻推拿促进NGF表达的最佳刺激参数。[结果]1.斜板实验造模7天后斜板实验数据分析,正常组、假手术组、模型组之间存在统计学差异(P0.01)。推拿干预20次后,推拿组(九组)之间采用正交试验的直观分析法,研究结果显示三因素的主次关系为:时间力量频率。最佳刺激参数组合是力量4N,时间1min,频率60次/min。推拿组(九组)之间采用方差分析法得出,三因素力量(A)、时间(B)、频率(C)对结果的影响无统计学差异(P0.05)。但从F值的大小判断(正交试验中对应因素的F值越大,该因素对结果的影响也越大),来看,可认为各因素的主次关系为:时间力量频率,而以短时间治疗效果为佳。这与直观分析法得出的结果一致。2.坐骨神经功能指数推拿干预20次后,推拿组(九组)之间采用正交试验的直观分析法,研究结果显示三因素的主次关系为:频率时间力量。最佳刺激参数组合是力量2N,时间1min,频率30次/mmin。推拿组(九组)之间采用方差分析法得出,三因素力量(A)、时间(B)、频率(C)对结果的影响无统计学差异(P0.05)。但从F值的大小判断(对应因素的F值越大,该因素对结果的影响也越大),来看,可认为各因素的主次关系为:频率时间力量,这与直观分析法得出的结果一致。3.腓肠肌湿重恢复率造模7天后,推拿组与模型组腓肠肌湿重恢复率无显著性差异(P0.05),推拿干预20次后,推拿组腓肠肌湿重恢复率测定结果与模型组比较有显著性差异(P0.05),代表腓肠肌萎缩后进行推拿干预可有预防肌肉萎缩的效果。推拿干预20次后,推拿组(九组)之间采用正交试验的直观分析法,研究结果显示三因素的主次关系为力量>频率时间,根据F值的大小,最佳刺激参数组合是力量4N,时间5mmin,频率30次/min。推拿组(九组)之间采用方差分析法得出,三因素力量(A)、时间(B)、频率(C)的主次关系无显著性差异(P0.05)。但从F值的大小判断(对应因素的F值越大,该因素对结果的影响也越大),来看,可认为各因素的主次关系为:力量频率时间,这与直观分析法得出的结果一致。4.脊髓腹角NGF光密度推拿干预20次后,推拿组(九组)之间采用正交试验的直观分析法,研究结果显示三因素的主次关系为:力量时间频率。最佳刺激参数组合是力量4N,时间1min,频率60次/min。推拿组(九组)之间采用方差分析法得出,三因素力量(A)、时间(B)、频率(C)对结果的影响有统计学差异(P0.05)。各因素的主次关系为力量时间频率,但从单因素来看,A(力量)因素、B(时间)因素的影响显著(P0.05),C(频率)因素的影响不显著(P0.05)。[结论]1.通过三法三穴干预可以促进SNI大鼠运动功能的恢复,刺激参数为中等力量、短时间、中等频率为佳。而手法力量过重、时间过长时疗效反而降低。2.通过三法三穴干预可以调控SNI大鼠脊髓腹角NGF的表达。通过NGF的提高可使运动功能恢复而刺激参数以中等力量、短时间、中频率为佳。
[Abstract]:[Objective] to compare the effect of behavior, gastrocnemius function and immuno histochemical related indexes under the guidance of orthogonal test, and study the influence of the strength, time and frequency of the manipulative parameters on the recovery of motor function in rats with peripheral nerve injury. With the sciatic nerve injury as an example, the peripheral nerve injury was studied with the sciatic nerve injury as an example. SD rats were used as experimental animals. The main and secondary relationships of all factors were power time frequency, but from the single factor, the influence of A (strength), B (time) factors was obvious (P0.05) and C (frequency) factors were not significant (P0.05). [conclusion]1. through three methods and three points. Intervention can promote the recovery of motor function of SNI rats. The stimulation parameters are medium strength, short time and moderate frequency. While the manipulative force is too heavy and the time is too long, the curative effect reduces the expression of NGF in the ventral horn of the spinal cord of SNI rats through the intervention of three methods and three points.
