益气健脾法治疗功能性腹泻脾胃虚弱证的临床观察

【摘要】：目的:通过对益气健脾法治疗功能性腹泻脾胃虚弱证的临床观察,评定其临床疗效。方法:纳入中医辨证属脾胃虚弱证的功能性腹泻患者共40例,治以益气健脾法,采用治疗前后自身对照的方法进行观察研究。本临床观察治疗时间为4周,分别记录患者治疗0周、治疗4周时的中医症状体征积分及心理量表评分,并应用SPSS19.0进行前后对比统计分析,以观察功能性腹泻患者的症状改善情况。于停药后12周对完成治疗观察的患者进行随访,通过观察患者症状体征积分相较于治疗4周时的变化情况判断其复发情况。结果:所纳入的40例患者中,脱落3例,其余37例顺利完成治疗观察,其中临床治愈8人,显效15人,有效12人,无效2人,总有效率为94.59%。对比37名患者治疗前、治疗后的中医症状总积分以及粪便不成形、完谷不化等各单项症状积分均显示P0.01,提示有显著差异;患者的SAS、SDS评分亦较疗前有所下降。在后续的随访过程中,脱落2例,复发4例,复发率为11.4%。结论:益气健脾法治疗功能性腹泻脾胃虚弱证具有显著的疗效,能明显改善患者各中医证候表现,且使患者的心理状态得到一定程度的改善,提高了其生活质量,同时益气健脾法治疗功能性腹泻尚具有复发率低、远期疗效较好的优势,有较好的临床应用前景。
[Abstract]:Objective: to evaluate the clinical efficacy of invigorating qi and invigorating spleen in the treatment of functional diarrhea with deficiency of spleen and stomach. Methods: 40 cases of functional diarrhea with syndrome differentiation of spleen and stomach were treated with the method of invigorating qi and invigorating spleen. The self-control method before and after treatment was used to observe and study. The clinical observation and treatment time was 4 weeks. The scores of TCM symptoms and signs and psychological scale were recorded after 0 weeks and 4 weeks of treatment respectively. SPSS 19.0 was used to carry out statistical analysis before and after treatment. To observe the improvement of symptoms in patients with functional diarrhea. The patients who completed the treatment observation were followed up at 12 weeks after withdrawal. The recurrence of the patients was judged by observing the scores of symptoms and signs of the patients compared with the changes of the symptoms and signs of the patients after 4 weeks of treatment. Results: among the 40 cases, 3 cases dropped off and 37 cases completed the observation successfully. Among them, 8 cases were cured, 15 cases were effective, 12 cases were effective and 2 cases were ineffective. The total effective rate was 94.59%. Compared with 37 patients before and after treatment, the total score of TCM symptoms and fecal dysplasia, and the scores of all individual symptoms showed P 0.01, indicating that there was significant difference, and the scores of SASN SDS were also lower than those before treatment. In the follow-up, 2 cases dropped off, 4 cases recurred, the recurrence rate was 11.4%. Conclusion: the method of invigorating qi and invigorating spleen is effective in treating functional diarrhea with deficiency of spleen and stomach. It can obviously improve the symptoms of TCM, improve the psychological state of patients and improve their quality of life. At the same time, the method of replenishing qi and strengthening spleen still has the advantages of low recurrence rate and good long-term curative effect, and has a good clinical application prospect.
