基于综合方法的中医临床实践指南制定程序研究

【摘要】：研究背景：中医临床诊疗以辩证论治为核心,诊疗方案动态,灵活,体现个体化和地域性特点,这样使得中医临床诊疗行为存在较大差异,优秀的诊疗技术难以推广也使其科学性受到质疑。中医临床实践指南作为规范中医诊疗行为、促进中医药推广应用的有力手段受到政府和专家的高度重视。白“十一五”期间至今,已发布了500余项中医临床实践指南,但许多指南的方法学质量普遍偏低,在实践使用中存在许多问题,缺乏符合中医诊疗特点的方法学工具是造成这种现象的主要原因。本研究旨在建立一套符合中医特点、清晰可操作的方法学工具,并在此基础上针对地区特点(香港),使用建立的方法学工具制定一份香港中医临床实践指南,在实践过程中验证本方法学的可行性。研究目的：1、建立一套基于综合方法的中医临床实践指南制定的方法学工具,帮助制定证据强、临床适用性高、更加符合国际规范的中医临床实践指南。2、基于综合方法的中医临床实践指南制定程序及方法学工具制定一份地区(香港)中医临床实践指南,探索方法学在实际应用中的问题。研究方法：首先,通过梳理国际上常用的临床实践指南制定手册、工具及相关方法,对不同临床实践指南制定程序中的关键要素进行总结抽提,结合定量分析、归纳比对等方法确定临床实践指南制定程序中必要环节。而后,选择具有代表性的高血压病中医临床实践指南为对象,采用文献计量学方法,分析2006-2010年期间文献中高血压病中医临床实践指南的使用情况及依从度；采用问卷调查法分析508名中医医师在临床实践中使用高血压病中医临床实践指南的基本情况和存在问题；通过分析高血压病中医临床实践指南在实际使用中存在的问题,找出中医临床实践指南存在的共性缺陷。针对共性问题设计出相应的方法学工具,并将方法学工具与临床实践指南制定程序的必要环节相结合,最终形成具特色的中医临床实践指南制定的基本程序及方法学工具。在此基础上,选取香港地区中医临床实践的地域背景、人群特征、用药习惯等,运用本研究提出的中医临床实践指南制定的方法学工具,按照规范流程,收集数据,综合证据制定出一份香港地区中医临床实践指南,以验证方法学的可行性。研究结果：本研究经过系统文献检索,共得到19份来自不同国家、地区、组织指南制定方法的相关工具或手册,11项关键要素被确定为临床实践指南制定的必要要素。通过高血压病中医临床实践指南在临床科研和临床实践中的使用情况分析,总结了中医临床实践指南可能存在的缺陷,主要包括缺乏规范的制定程序、缺乏科学证据、缺乏临床适用性、缺乏规范的报道。通过分析这4项主要缺陷分析的成因,从方法学角度提出了改进意见。在此基础上本文提出了中医临床实践指南制定的基本程序及方法学工具,主要包括3个阶段6个步骤,证据综合表格及中医临床实践指南报道规范。针对缺乏规范制定程序的问题,将中医临床实践指南制定的方法规范为清晰可操作的步骤流程,并在不同步骤下提供可操作的方法学工具,加强操作性和依从性；针对缺乏证据支持问题,采用循证方法、数据挖掘、德尔菲法收集证据并进行证据综合,最大程度上加强指南证据的力度；针对缺乏临床适用性问题,研究突破传统意义上“临床证据”的概念,将本地专家经验、用药习惯、中医诊疗背景等信息作为重要证据融入指南内容；针对缺乏规范报道的问题,本文给出了10个领域19个条目的中医临床实践指南报告规范,并对每个领域及条目,做出了详细的解释说明,以便于清晰规范的报道指南内容。最后,选择香港地区常见病慢性胃炎为对象,遵循中医临床实践指南制定的基本程序,开展中医临床实践指南的制定工作。在第一阶段,准备阶段),通过组建指南的起草委员会,确定了工作职责和工作计划。第二阶段,指南的制定阶段。首先,从3个方面收集了证据,通过文献检索定位了6份慢性胃炎的中医临床实践指南,用AGREEII进行质量评价,筛选出3份质量较高的指南,对其中的证据条目进行拆分,作为文献证据来源；使用数据挖掘方法,对慢性胃炎的中医证型、中药方剂、针灸穴位等信息进行提取；采用德尔菲法,收集本地中医医师治疗慢性胃炎的经验意见、现有治疗方案与本地法律背景、诊疗环境的适用程度等信息。然后,将三方面的证据进行综合,形成慢性胃炎中医临床实践指南的基本条目内容,每个条目给出对应的支持证据。最后,通过正式专家共识会议法,判断进入中医临床实践指南的条目内容。最后,按照规范报告格式的条目,起草中英版指南。第三阶段,完成阶段,制定了指南的更新计划、实施方案。
[Abstract]:This study aims to establish a set of guidelines for clinical practice of Chinese medicine based on comprehensive methods , and to establish a set of guidelines for clinical practice of TCM clinical practice . The aim of this study is to establish a set of guidelines for clinical practice of TCM in TCM .
A questionnaire was used to analyze the basic situation and existing problems of 508 Chinese medicine practitioners in clinical practice .
This paper analyzes the causes of clinical practice in Hong Kong , and puts forward some basic procedures and methodological tools for the clinical practice guideline of TCM .
In view of the lack of evidence support , evidence - based approach , data mining , Delphi method is used to collect evidence and to conduct evidence synthesis , to maximize the strength of the guidance evidence ;
In view of the lack of clinical applicability , the author studies the concept of " clinical evidence " in the traditional sense , and integrates the experience of local experts , the habit of medication , the background of medical treatment and treatment as important evidence into the guidance content ;
In order to solve the problem of lack of normative reporting , this paper gives a detailed explanation of the clinical practice guidelines of TCM clinical practice of 19 items in 10 areas , and makes detailed explanation on each area and entry . Then , the three kinds of evidence are combined to form the basic item contents of the clinical practice guide of chronic gastritis . Finally , according to the formal expert consensus conference method , the contents of entry into Chinese clinical practice guide are judged . Finally , according to the format of the standard report , the Chinese - English guide is drafted . At the third stage , the completion stage , the updated plan and implementation plan of the guide are developed .
