针药结合对盆腔炎性疾病后遗症大鼠血清中IL-6、IL-8表达的影响

【摘要】:目的:通过观察针刺结合盆舒饮对盆腔炎性疾病后遗症大鼠血清中促炎性细胞因子IL-6、IL-8的含量影响,探讨其治疗盆腔炎性疾病后遗症的作用机制及效果优势。方法:1、选用Wistar雌性大鼠作为受试对象,采用"机械损伤+混合菌感染"的方法建立盆腔炎性疾病后遗症大鼠模型。分别于造模后第7、14、21天对大鼠子宫组织进行病理学观察。2、随机抽取8只大鼠作为空白组,将其余大鼠进行造模,造模后的大鼠随机分成5组,于造模后第15天进行干预治疗。空白对照组和模型组都给予蒸馏水灌胃,针刺组选取腧穴关元、血海和三阴交进行治疗,中成药组以丹黄去瘀胶囊悬液灌胃,中药组以盆舒饮汤剂进行灌胃、针药结合组大鼠给予针刺刺激同时联合盆舒饮灌胃治疗,14天后,采用HE染色法,通过光镜对大鼠子宫组织病理改变进行观察;用ELISA法检测大鼠血清中的促炎性因子IL-6、IL-8的浓度。结果:1、成功建立盆腔炎性疾病后遗症大鼠模型。通过观察发现:造模术后7天,大鼠子宫组织明显充血,炎细胞浸润,无盆腔粘连、纤维化改变等典型慢性炎症表现;造模术后14天,肉眼观察大鼠盆腔组织广泛粘连,子宫积水严重,光镜下可见腺体扩张,纤维化改变明显;IL-6、IL-8在SPID模型大鼠血清中的表达明显高于正常大鼠,p0.01。2、经过两周干预治疗后,各治疗组大鼠盆腔情况均有不同程度好转,其中以针刺结合盆舒饮组改善程度最佳;各治疗组大鼠血清中IL-6、IL-8水平均有所下调,其中以针刺联合盆舒饮组下降最明显,与正常对照组比较,p0.05。结论:通过"机械损伤+混合菌感染"的方法可成功建立SPID大鼠模型。针刺联合盆舒饮主要通过抑制炎症大鼠血清中的促炎性细胞因子IL-6、IL-8的表达,来减轻组织损伤,促进炎症吸收,从而达到治疗盆腔炎性疾病后遗症的目的。
[Abstract]:Objective: to observe the effect of acupuncture combined with Pan-Shuyin on the content of IL-6 and IL-8 in the serum of rats with pelvic inflammatory disease sequelae, and to explore the mechanism and advantages of the treatment of pelvic inflammatory disease sequelae. Methods the female Wistar rats were selected as subjects, and the model of pelvic inflammatory disease sequelae was established by the method of "mechanical injury mixed bacteria infection". The uterine tissues of rats were observed by pathological observation on day 7, 14, 21, respectively. 8 rats were randomly selected as blank group. The other rats were randomly divided into 5 groups, and the rats were treated with intervention on the 15th day after the model. The blank control group and the model group were given distilled water, the acupuncture group was treated with acupoint Guanyuan, Xuehai and Sanyinjiao, the traditional Chinese medicine group was perfused with Danhuangzaiyu capsule suspension, and the traditional Chinese medicine group was perfused with Pan-Shuyin decoction. After 14 days of acupuncture stimulation combined with gastric perfusion, the pathological changes of rat uterus were observed by HE staining, and the concentration of IL-6 and IL-8 in serum of rats was detected by Elisa. Results the rat model of pelvic inflammatory disease sequelae was established successfully. It was found that there were obvious hyperemia, inflammatory cell infiltration, no pelvic adhesion, fibrosis and other typical chronic inflammatory manifestations in rat uterus 7 days after modeling, and extensive adhesions of pelvic tissue were observed with naked eye on 14 days after model making. The uterine hydronephrosis was serious, glandular dilatation was observed under light microscope, and the expression of IL-6 IL-8 in serum of SPID model rats was significantly higher than that of normal rats. After two weeks of intervention, the pelvic conditions of each treatment group were improved to some extent. The level of IL-6 and IL-8 in serum of rats in each treatment group was down-regulated, especially in the acupuncture combined with Pan-Shuyin group, which was significantly lower than that in the normal control group (P 0.05). Conclusion: SPID rat model can be established successfully by the method of "mixed strain of mechanical injury". Acupuncture combined with Pan-Shuyin can reduce tissue injury and promote inflammatory absorption by inhibiting the expression of IL-6 and IL-8 in the serum of inflammatory rats so as to achieve the purpose of treating the sequelae of pelvic inflammatory disease.
