基于手功能成像仪针刺治疗脑卒中后手运动功能障碍的疗效评价

【摘要】：目的研究手功能激光成像仪应用于针刺治疗脑卒中后手功能的评价。方法 2016年1月至6月,18例脑卒中后上肢功能障碍患者分为对照组(n=9)和治疗组(n=9),对照组接受常规药物治疗和康复训练,治疗组在此基础上接受针刺治疗。治疗前及治疗3周后,两组患者采用改良Ashworth量表(MAS)、Fugl-Meyer评定(FMA)上肢运动功能部分、改良Bathel指数(MBI)进行评价,采用手功能激光成像仪进行感兴趣区(ROI)测量。结果治疗后,MAS、FMA评分明显改善(t4.44,P0.01),治疗组BI评分明显改善(t=3.95,P0.01),治疗组MAS、FMA、BI评分均明显优于对照组(t3.17,P0.01)。两组治疗后,患手ROI评分均显著降低(t7.78,P0.001),治疗组低于对照组(t=-2.23,P0.05);两组间健手ROI评分无显著性差异(t=-0.11,P0.05)。结论手功能激光成像可以提供手部血流灌注情况的信息,与手功能有一定关联。
[Abstract]:Objective to study the application of hand function laser imager in the evaluation of hand function after acupuncture treatment of stroke. Methods from January to June 2016, 18 patients with upper limb dysfunction after stroke were divided into two groups: control group (n = 9) and treatment group (n = 9). The control group received routine drug therapy and rehabilitation training, and the treatment group received acupuncture treatment on this basis. Before treatment and 3 weeks after treatment, the patients in both groups were evaluated with modified Ashworth scale (Mas) and modified Bathel Index (MBI), and the area of interest (ROI) was measured by hand function laser imager. Results after treatment, the FMA score of MASA was significantly improved (t 4.44 卤P0.01), the BI score of the treatment group was improved significantly (t = 3.95, P 0.01), and that of the treatment group was significantly better than that of the control group (t 3.17, P 0.01). After treatment, the ROI scores of the affected hands decreased significantly (t7.78 卤P0.001), which were lower in the treatment group than in the control group (t = 2.23 P 0.05), but there was no significant difference between the two groups in the ROI scores of the healthy hands (t = -0.11 P 0.05). Conclusion the laser imaging of hand function can provide the information of blood flow perfusion in hand, which is related to hand function.
