清脑益元汤对缺血性中风患者急性期NSE、S100B表达的影响

【摘要】:目的:观察清脑益元汤对缺血性中风急性期血清NSE、S100B表达的影响,探讨其对缺血性中风患者的治疗效果及作用机制。方法:采取随机原则,选取广西中医药大学第一附属医院脑病科从2016年04月至2016年12月的住院患者共60例,随机分为治疗组与对照组,每组各30例。对照组予以单纯西医常规治疗,治疗组在对照组基础上加用清脑益元汤。采用酶联免疫吸附法(ELISA)检测两组治疗前及治疗后第3天、第7天、第14天血清NSE、S100B表达水平,评估两组患者神经功能缺损程度评分、中医证候积分、生活能力状态及临床疗效。结果:(1)治疗前,两组神经功能缺损评分、中医临床证候积分、生活能力状态及血清NSE、S100B水平比较,差异均无统计学意义(P0.05);(2)治疗后,治疗组与对照组神经功能缺损评分、中医临床证候积分及血清NSE、S100B表达水平比较均较前下降,且治疗组下降更明显(P0.05或P0.01);(3)两组治疗前神经功能积分与血清NSE、S100B表达水平呈正相关(相关系数:NSE:r=0.925,r=0.958;S100B:r=0.891,r=0.870,P均0.01),血清NSE与S100B表达水呈正相关(两组相关系数:r=0.981,r=0.870,P均0.01)。(4)生活能力状态较前得到明显改善(P0.01),治疗组改善更显著(P0.05)。治疗组临床总有效率明显高于对照组,两组临床总有效率分别为90%和76.7%,差异有统计学意义(P0.05)。结论:清脑益元汤能明显减轻缺血性中风患者急性期神经功能缺损程度,其机制可能与下调血清NSE、S100B表达水平有关。清脑益元汤值得在临床推广应用。
[Abstract]:Objective: to observe the effect of Qingnao Yiyuan decoction (QNYD) on the expression of serum NSE S100B in acute ischemic stroke, and to explore the therapeutic effect and mechanism of Qingnao Yiyuan decoction on ischemic apoplexy. Methods: a total of 60 inpatients from the Department of Encephalopathy in the first affiliated Hospital of Guangxi University of traditional Chinese Medicine from April 2016 to December 2016 were randomly divided into treatment group and control group with 30 cases in each group. The control group was treated with routine western medicine and the treatment group was treated with Qingnao Yiyuan decoction on the basis of the control group. Enzyme linked immunosorbent assay (Elisa) was used to detect the serum NSES100B expression before treatment and on the 3rd, 7th and 14th day after treatment, and to evaluate the neurological deficit score, TCM syndromes score, living ability status and clinical efficacy of the two groups. Results: (1) before treatment, there was no significant difference between the two groups in neurological deficit score, TCM clinical syndrome score, living ability status and serum NSE S100B level (P0.05). After treatment, the nerve function defect scores of the treatment group and the control group were not significantly different (P0.05), and no significant difference was found between the treatment group and the control group (P0.05). The scores of TCM clinical syndrome and the expression level of serum NSEP S100B were lower than those before. In the treatment group, there was a more significant decrease (P0.05 or P0.01); (3). There was a positive correlation between the scores of nerve function and the expression of serum NSE S100B in the two groups (the correlation coefficient was 0: 0. 925); (0. 925), the correlation coefficient was 0. 858 S0. 891B: R0. 870 (P0.01), and the serum NSE was positively correlated with the water expression of S100B (the correlation coefficient of the two groups was 0. 981rngr, 0. 981rr, P 0. 01). (4), and there was a positive correlation between the expression of serum NSE and the expression of S100B in the treatment group (P < 0. 01). The state of the treatment group was significantly improved (P0.01), and the improvement of the treatment group was more significant (P0.05). The total clinical effective rate of the treatment group was significantly higher than that of the control group, the total effective rate of the two groups was 90% and 76.7% respectively, the difference was statistically significant (P0.05). Conclusion: Qingnao Yiyuan decoction can obviously reduce the degree of nerve function defect in patients with ischemic stroke in acute stage, and its mechanism may be related to the down-regulation of serum NSE S100B expression level. Qingnao Yiyuan decoction is worth popularizing in clinic.
