王希浩主任医师辨治绝经前后诸证学术思想和临床经验探讨

【摘要】：目的:1绝经前后诸证是妇科常见病,也属疑难杂症。近年来本病发病率逐年上升,不仅严重危害妇女身心健康,同时也给家庭和社会造成极大影响。王希浩导师是国家级第五批名老中医药专家师承指导老师,硕士生导师,主要从事中医妇科临床与科研工作。导师在中医药治疗女性内分泌与生殖免疫调节方面有深入研究,尤其对绝经前后诸证的辨证和治疗,经验独到,但其许多宝贵经验未能及时整理归纳。本文通过对导师的讲课内容、论文集著、门诊病例跟踪观察,结合个人临床体会,全面系统地进行归纳整理,进而探讨导师辨治绝经前后诸证的学术思想,以期为祖国医学事业的发展略尽绵力。2绝经前后诸证临床表现纷繁,如烘热汗出、烦躁易怒、心悸失眠、潮热面红、眩晕耳鸣、面浮肢肿、腰背酸楚、皮肤蚁行感、情志不宁等症状,中医药治疗绝经前后诸证虽然疗效确切,副作用少,但在病因病机、辨证分型、疗效评定等方面尚缺乏统一标准。本文旨在通过对王希浩导师治疗绝经前后诸证的学术思想的探讨总结,以期获得绝经前后诸证的病因病机、辨证分型、疗效评定等方面的量化指标,以便临床推广应用。方法:1通过汇总王希浩导师学术论文30余篇,研析导师讲课资料、学术论著等,对导师辨治绝经前后诸证的方法和用药特点进行探讨分析和归纳总结。2整理其临证医案150余例,自拟方、经验方100余首,对方内随症加减药物及使用频次进行统计分析。3查阅文献,深度分析导师学术思想和现有治疗妇科疾病理论框架之间的关系。4结合个人临床体会和学术交流,对导师学术思想进行实践学习和反复验证。5系统总结出导师辨治绝经前后诸证的学术思想,书写专业论文。成果:跟师过程中,搜集汇总导师学术论文30余篇,整理其临证医案150余例,自拟方、经验方100余首,研析其讲课资料、学术论著等,对导师辨治绝经前后诸证的方法和用药特点进行探讨分析和归纳总结,并结合个人临床体会,初步整理总结出导师对绝经前后诸证的辨证思路及诊疗规律。具体归纳为以下几方面:1导师对绝经前后诸证病因病机的认识1.1“肾阴亏虚是其本”。祖国医学认为,妇女“七七”之年,肾气渐衰,天癸将竭,冲任二脉亏损,精血不足,月经逐渐减少而至绝经,生殖能力逐渐下降而至消失。导师认为,女子属阴,妇女一生历经“经、孕、产、乳”,数次耗伤阴血,机体处于“阴常不足,阳常有余”的生理状态,又因“年逾四十而阴气自半”,若素体阴虚或房劳多产;或情志内伤,气火伤阴;或大病久病,穷必及肾等,肾阴就会更加亏虚,不能灌溉五脏,滋养诸经,进而变生诸症。所以肾阴亏损是本病的发病之本。1.2“阳亢火炎是其标”。中医认为,心肾属水火既济关系,肾肝属于精血同源关系。绝经前后肾阴亏虚不能上济于心,可使心火独亢,出现心肾不交、水火失济,临床常见面部烘热、心悸、心慌以及失眠、烦躁、焦虑、抑郁等心火上炎症状;肾阴亏虚,肾水不足以涵养肝木,肝失柔养,临床常见眩晕耳鸣、腰膝酸软、心烦急躁、咽干口苦、心悸健忘、失眠多梦等一系列肝肾阴虚、肝阳上亢症状。可见,绝经前后诸证以肾阴亏虚为本,由肾阴亏虚所致的阳亢火炎是其常见表象。1.3“病情涉及心肝脾”。妇女绝经前后,肾气虚衰、阴阳失调为本,又常常波及心肝脾三脏。波及于心,可见心阴亏虚,心肾不交证;波及于肝,常出现肝阴不足、肝阳上亢和肝经郁热化火的症状表现;波及于脾,常见脾肾阳虚、脾胃虚弱等证。2导师辨治绝经前后诸证的临床经验2.1“补肾为主,兼调心肝脾”2.1.1“治病求本,补肾养阴是关键”。绝经前后的妇女由于体质差异,疾病、产育、营养等因素,再加上家庭问题、社会环境、工作压力等方面的原因,使得机体处于“阴常不足,阳常有余”的病理状态。肾阴亏虚,脏腑失养,继而出现绝经前后诸证如潮热、盗汗以及情绪障碍如抑郁、焦虑等。因此肾阴亏虚是绝经前后诸证的基本病机。治疗应以滋补肾阴、填精益髓为基本大法。2.1.2“兼顾他脏,心肝脾各有侧重”。绝经前后诸证的基本病机是“其本在肾,涉及心肝脾”,在本证中,除肾阴亏虚的主要临床表现外,常可见偏肝、偏心、偏脾的不同症候。所以,导师在临证中,十分重视“审查病机”,严格做到“有是症用是药”。治疗上,除滋补肾阴为主外,又根据临床所出现的偏于心、肝、脾的不同症状,用药各有侧重。2.2“平调阴阳,善于阴中求阳、阳中求阴”。导师辨证治疗绝经前后诸证,从肾虚入手,平调肾中阴阳。虽然本病肾阴亏虚证居多数,但也不忽视肾阳虚和肾阴阳两虚证。治疗上注意温肾阳和阴阳双补,并善于阳中求阴,阴中求阳,滋阴之中不忘助阳。2.3“心理疏导,重视人文关怀”。绝经前后诸证作为一种身心疾病已被大家公认,因此心理疗法本病的治疗具有重要的作用。中医认为情志是五脏功能的外在表现,只有脏腑功能正常,情志才会正常,反过来五志过极及七情失常又可影响脏腑的功能。导师在临证时尤其重视人文关怀,在药物治疗的基础上,常常给予患者必要的心理疏导及健康教育,以帮助患者顺利渡过绝经期。2.4“标本兼顾,从虚、热、瘀辨治绝经期功血”。绝经期功血,是指妇女绝经前后因卵巢机能减退,卵泡几乎耗竭,不能正常排卵引起的月经周期、经期、经量紊乱。此病是妇科临床常见病、多发病,也是疑难急重病症,属中医学崩漏范畴。导师继《内经》之理论,承傅青主之学说,结合数十年临床实践,认为绝经前后崩漏的病因病机主要是阴虚内热,热迫血行,兼有瘀血瘀毒。治疗以固肾、调肝、健脾为主,辅以清热凉血止血、化瘀解毒。3导师治疗绝经前后诸证的用药规律收集2012年9月至2015年5月导师治疗绝经前后诸证病例56份,处方172张,并统计出其中常用药物的使用频次和频率。结果显示,处方中药物出现频率具有统计学意义的有28味,占统计处方中药物频率的71.44%,而这些药物正是组成治疗本病的基本方(黄柏、知母、生熟地、生山药、山萸肉、当归、白芍、川楝子、生龙骨、生牡蛎、丹参、鸡内金、白术)及兼证常用方的主要药物,可以看出导师治疗绝经前后诸证主要从滋补肾阴、兼调心肝脾入手,在本证本方的基础上随证加减。结论:绝经前后诸证是由于妇女“七七”之年肾气渐衰,天癸将竭,冲任虚损,精血不足,进而导致机体阴阳失调,脏腑功能紊乱。导师从事妇科临床与科研三十余载,对本病的辨证施治积累了极其丰厚的临床经验,认为本病的基本病机是“其本在肾,涉及心肝脾”,治疗以滋补肾阴为主,兼调心肝脾,同时注意平衡阴阳,标本兼顾。临床辨证准确,用药精当,疗效显著。
