放血拔罐联合中药清眩润目饮内外并用治疗MGD所致干眼的临床观察

【摘要】：目的:通过临床观察,研究放血拔罐联合中药清眩润目饮内外并用治疗MGD所致干眼的疗效。方法:本课题采用随机分组法将90例干眼患者分为3组(每组30例)。对照组1(中药组)采用中药清眩润目饮内外并用治疗,对照组2(西药配合睑板腺按摩组)采用普拉洛芬滴眼液、玻璃酸钠滴眼液联合加替沙星眼用凝胶局部点眼配合睑板腺按摩,治疗组(放血拔罐联合中药组)放血拔罐联合中药清眩润目饮内外并用治疗。三组均以4周为疗程。对比观察治疗前后患者的主观症状积分(OSDI),泪液分泌试验(SIT),泪膜破裂时间(BUT),睑板腺功能检查,角膜荧光素染色(FCS),中医主症的指标,进行优效性检验。结果:1.三组患者治疗前后主观症状积分(OSDI),泪液分泌试验(SIT),泪膜破裂时间(BUT),睑板腺功能检查,角膜荧光素染色(FCS),中医主症均有改善。治疗组各项指标治疗前后改善程度明显,且有显著性意义(p0.01)。2.在主观症状积分方面,治疗前后差值组间比较,治疗组优于两对照组,治疗组分别与对照组1,对照组2比较,具有统计学意义(p0.05)。3.在泪膜破裂时间(BUT)方面,运用LSD法对三组间治疗前后差值进行两两比较结果显示,治疗组疗效优于两对照组,且差异有统计学意义(P0.05);对照组1和对照组2差异无统计学意义(P=0.5270.05)。4.在泪液分泌试验(Schirmer试验)方面,三组治疗前后差值比较,总体差异有统计学意义(P0.05)。治疗组疗效优于对照组1和对照组2,且有统计学意义;对照组1优于对照组2,但无统计学意义。5.在睑板腺功能检查方面,治疗前后组间差值比较,治疗组优于两对照组(P0.05),对照组1疗效的虽高于对照组2,但两组比较差异无统计学意义(P0.05)。6.在角膜荧光素染色方面,治疗前后差值组间比较,治疗组效果优于两对照组(P0.05)。治疗组与对照组1和对照组2相比较,均有统计学意义(P0.05)。但两对照组差异无统计学意义。7.在中医主症方面,三组治疗前后差值比较,总体差异有统计学意义(P0.05)。运用LSD法对三组间治疗前后差值进行两两比较结果显示,治疗组明显高于对照组1、对照组2,差异有统计学意义(P=0.0000.05);对照组1和对照组2差异无统计学意义(P=0.1720.05)。说明治疗组治疗效果优于对照组1和对照组2,且有统计学意义;对照组1优于对照组2,但无统计学意义。结论:1.放血拔罐联合中药清眩润目饮内外并用、西药联合睑板腺按摩、中药清眩润目饮内外并用对MGD所致干眼均具有治疗效果,三者均能改善患者的自觉症状、减轻眼表炎症,增加泪膜的稳定性,促进泪液分泌。2.在改善睑板腺功能方面,放血拔罐联合中药清眩润目饮内外并用具有显著优势。3.在改善中医症状方面,放血拔罐联合中药清眩润目饮内外并用疗效甚佳。4.放血拔罐联合中药清眩润目饮内外并用治疗MGD所致干眼的疗效明显优于西药联合睑板腺按摩和中药清眩润目饮内外并用。而单纯中药清眩润目饮内外并用优于西药联合睑板腺按摩。
[Abstract]:Objective: through clinical observation, the effect of bleeding and cupping combined with Chinese medicine Qing Xuan moisten eye drink inside and outside the treatment of dry eye caused by MGD.
