“培土生金法”结合针刺治疗常年性变应性鼻炎的临床研究

【摘要】：目的:变应性鼻炎是以鼻粘膜病变为主的变态反应性疾病,是耳鼻喉科的常见病之一,患者常常在接触过敏原后出现鼻痒、阵发性鼻塞、连续喷嚏、流涕等症状。目前发病率不断升高,常年性变应性鼻炎患者病情反复发作,迁延不愈,严重者甚至合并或诱发哮喘等呼吸系统疾病。西医药物治疗在短期内能改善症状,但长期服用副作用明显,不能从根本上改善患者的过敏体质;免疫治疗、手术治疗费用较高,不能被广泛接受。中医针灸在治疗上具有抗过敏、消炎、促进血液循环、提高机体防御能力等疗效,同时具有安全简便、疗效确切、副作用小的优点。通过针刺特定的腧穴以补益脾胃,进而达到增强肺气、疏通鼻窍、缓解症状、减少复发的效果。本研究通过观察"培土生金法"结合针刺治疗常年性变应性鼻炎的临床疗效,探究脾胃之气的强弱对于常年性变应性鼻炎的影响及其机制,为临床治疗变应性鼻炎患者提供更有效的治疗方法,丰富理论认识,为实验研究提供一定的临床依据。方法:本研究根据诊断标准、纳入标准、排除标准,选取2016年6月至2017年3月在广州中医药大学第一附属医院耳鼻喉科门诊、针灸科门诊就诊的患者。按照随机对照原则,将60名患者分为治疗组和对照组,治疗组30例患者针刺以鼻局部腧穴、脾俞、胃俞、足三里为主治疗,对照组30例患者针刺以鼻局部腧穴配合列缺、肺俞为主,结合对症选穴。每周治疗3次,10次为一个疗程,共治3个疗程。通过观察鼻症状积分、鼻体征积分、鼻结膜炎相关生活质量评分,治疗3个疗程后,评估两组疗效、患者鼻症状体征总积分的变化情况;治疗结束后1月随访,比较两组鼻炎生活质量积分。运用统计软件SPSS17.0进行数据统计处理,得出相应的试验结果及结论。结果:1.比较治疗后两组疗效,结果治疗组显效6例,有效20例,无效4例,总有效率为86.7%,对照组显效2例,有效15例,无效13例,总有效率56.7%,经统计学分析,P0.05,两组总有效率差异具有统计学意义,说明治疗组疗效优于对照组;2.比较两组治疗前、后鼻症状体征总积分,经统计学分析,P0.05,差异具有统计学意义,说明两组均有临床疗效;比较治疗后两组鼻症状体征总积分,经统计学分析,P0.05,说明治疗组疗效优于对照组;3.比较两组治疗前、治疗结束后1月随访鼻炎患者生活质量症状评分,经统计学分析,P0.05,差异具有统计学意义,表明两组均有临床疗效;比较治疗后两组鼻言患者生活质量总评分,经统计学分析,P0.05,说明治疗组疗效优于对照组。结论:1.治疗后治疗组和对照组患者鼻症状体征较治疗前均有改善,表明二者均有助于治疗变应性鼻炎,但治疗组在改善鼻症状体征方面优于对照组,说明治疗组疗效优于对照组;2.治疗结束后1月随访,两组对于改善鼻炎患者生活质量均有疗效,但治疗组在改善鼻炎生活质量方面明显优于对照组,说明治疗组在提高患者患者生活质量、稳定疗效上优于对照组,临床上值得推广使用。3.综上所述,"培土生金法"结合针刺治疗常年性变应性鼻炎对于改善患者鼻塞、流涕、鼻痒、喷嚏均有显著效果,且无明显不良反应,有效的提高了鼻炎患者的生活质量,疗效稳定,但本研究仍需要进一步完善改进。
