参芪益肾汤治疗气阴两虚兼痰瘀型糖尿病肾病的临床观察

【摘要】：目的临床观察参芪益肾汤治疗糖尿病肾病前后期间,患者的各项指标变化和治疗效果。方法此次临床研究选取60例糖尿病肾病患者,并随机划分为治疗组和对照组各30例患者,符合临床纳入标准要求,及中医辨证分型标准。在基础治疗上,治疗组服用参芪益肾汤和对照组服用厄贝沙坦片,疗程均为16周。观察患者血常规、尿常规、肝功、肾功、血脂、血糖、糖化血红蛋白、24小时尿蛋白定量、尿微量白蛋白、氧化低密度脂蛋白受体的影响及中医证候积分进行统计学分析。结果受试患者通过口服参芪益肾汤后提高了治疗糖尿病肾病临床效果。临床治疗组有效率达到83.32%,而对照组有效率达到63.32%,治疗组有效率高于对照组(P0.01);患者证候积分均较治疗前明显降低(P0.01),治疗组明显优于对照组(P0.01);患者服药后24小时尿蛋白定量、尿微量白蛋白明显下降,治疗组优于对照组(P0.05);甘油三酯下降(P0.01);总胆固醇下降(P0.05);血糖下降(P0.05);糖化血红蛋白下降(P0.01);氧化低密度脂蛋白受体降低(P0.05);治疗后两组未出现生化指标改变。结论1.临床应用参芪益肾汤治疗气阴两虚兼痰瘀型糖尿病肾病的有效率、改善中医证候明显优于对照组。2.受试患者服药后能使糖尿病肾病患者Ⅲ期的24小时尿白蛋白定量、尿微量白蛋白、氧化低密度脂蛋白受体、指标下降。3.可以调节血糖和脂类代谢紊乱。4.减慢糖尿病肾病病情发展。
[Abstract]:Objective To observe the changes and therapeutic effects of Shenqi Yishen Decoction (Shenqi Yishi Decoction) in the treatment of diabetic nephropathy. Methods 60 patients with diabetic nephropathy were selected and divided into 30 patients in the treatment group and the control group, which were in accordance with the clinical inclusion criteria and the standard of TCM syndrome differentiation. On the basis of basic treatment, The treatment group took Shenqi Yishi Decoction and the control group to take Irbesartan Tablets for 16 weeks. The blood routine, urine routine, liver function, kidney work, blood lipid, blood glucose, glycemic hemoglobin, 24 hour urine protein quantitative, urine microalbuminuria, oxidized low density lipoprotein receptor and TCM syndrome score were statistically analyzed. The effective rate of the treatment group was 83.32%, the effective rate of the control group was 83.32%, the control group was 63.32%, the effective rate of the treatment group was higher than that of the control group (P0.01); the syndrome score of the patients was significantly lower than that before the treatment (P0.01), and the treatment group was obviously superior to the control group (P0.01), and the patient was 24 after taking the medicine. Hourly urine protein quantitative, urine microalbumin decreased significantly, the treatment group was better than the control group (P0.05), triglyceride drop (P0.01), total cholesterol drop (P0.05), blood glucose drop (P0.05), glycated hemoglobin (P0.01) and low density lipoprotein receptor reduction (P0.05); the two groups did not appear biochemical changes after treatment. Conclusion 1. clinical application of the conclusion The effective rate of Qi Yishen Decoction in the treatment of diabetic nephropathy with Qi Yin deficiency and phlegm stasis type diabetic nephropathy is better than that of the control group. The 24 hours urine albumin quantitation, urine microalbumin, oxidized low density lipoprotein receptor, and the decrease of.3. can regulate blood glucose and lipid metabolism turbulence in the patients with diabetic nephropathy after the treatment of.2. patients. The disorder of.4. slowed the development of diabetic nephropathy.
