电针治疗膝骨关节炎疗效性的META分析

【摘要】:背景:膝关节骨性关节炎,又称退行性膝骨关节病,增生性膝骨关节炎等,是一种多发于中老年人的慢性骨关节疾患。现代医学研究证明,膝关节的骨质和周围软组织退变程度随年龄的增长而加剧,高度的致畸和致残率,严重影响患者的生活质量。目前,临床上治疗方法有很多种,取得了一定的治疗效果。但由于膝骨关节炎的病因病机仍处于不明朗的状况,所以难以达到理想的治疗效果,西医的药物治疗又伴有不同程度的不良反应,所以寻找经济安全的治疗方法成为了社会的关注焦点。针刺腧穴有通经络、补气血、消肿止痛的功效,针刺加电针被证实可以活跃局部组织血液循环、抑制炎性物质发展,同时提高局部组织的痛阈,从而减轻疼痛等临床症状。国内多数临床文献认为针刺或电针对膝骨关节炎的治疗疗效乐观,但国际上对此仍有诸多看法。为探讨针刺及电针疗法对膝骨关节炎的临床疗效,有必要对相关文献进行科学、系统的评价与分析,得出结论,更客观的指导本病的临床治疗。目的:对电针治疗膝骨关节炎的临床疗效研究这一课题的多项独立临床研究进行Meta分析,希望通过探讨电针治疗膝骨关节炎的有效率及不良反应发生率,可以为膝关节骨关节炎的临床治疗的研究提供循证依据,并可以为临床指导治疗提供证据。方法:计算机检索CBM、CNKI、万方、维普、Pub Med和The Cochrane Library,搜集电针治疗膝骨关节炎的随机对照试验。使用Revman 5.3软件对数据进行Meta分析。结果:本分析纳入随机对照试验共13篇,实验组均为电针治疗,对照组均为西药治疗。一篇为口服扶他林缓释片,一篇为口服美洛昔康,一篇为口服盐酸氨基葡萄糖片,一篇为口服芬必得,两篇为双氯芬酸钠,三篇为布洛芬,一篇为塞来昔布,一篇为壮骨关节丸,两篇为口服扶他林或芬必得。Meta分析结果显示:电针治疗膝骨关节炎疗效性与安全性与药物相比,具有统计学意义。结论:电针治疗膝骨关节炎,较药物组治疗疗效显著,且具有安全性。
[Abstract]:Background: knee osteoarthritis, also known as degenerative knee osteoarthritis, hyperplastic knee osteoarthritis, is a kind of chronic osteoarthritis in middle and old people. Modern medical research has proved that the degree of degeneration of bone and surrounding soft tissue of knee joint increases with age, and the high rate of teratogenicity and disability seriously affects the quality of life of patients. At present, there are many kinds of clinical treatment methods, and have achieved certain therapeutic effects. However, because the etiology and pathogenesis of knee osteoarthritis are still in an uncertain state, it is difficult to achieve an ideal therapeutic effect. Western medicine treatment is accompanied by varying degrees of adverse reactions. Therefore, the search for economic safety treatment has become the focus of social attention. Acupuncture acupoints have the effect of opening meridian, replenishing qi and blood, reducing swelling and relieving pain. Acupuncture plus electroacupuncture has been proved to be able to activate blood circulation of local tissues, inhibit the development of inflammatory substances, and increase the pain threshold of local tissues, thereby alleviating the clinical symptoms such as pain and so on. Most clinical literatures at home think that acupuncture or electroacupuncture is an optimistic treatment for knee osteoarthritis, but there are still many views on it internationally. In order to study the clinical effect of acupuncture and electroacupuncture on knee osteoarthritis, it is necessary to make a scientific, systematic evaluation and analysis of the relevant literature, and draw a conclusion, which is more objective to guide the clinical treatment of this disease. Objective: to study the clinical efficacy of electroacupuncture in the treatment of knee osteoarthritis by Meta analysis in order to explore the effective rate and adverse reaction rate of electroacupuncture in treating knee osteoarthritis. It can provide evidence basis for clinical treatment of knee osteoarthritis and provide evidence for clinical treatment. Methods: a randomized controlled trial was conducted to search CBM,CNKI, Wanfang, Weipub Med and The Cochrane Library, for the treatment of knee osteoarthritis by electroacupuncture. The data were analyzed by Meta using Revman 5.3 software. Results: there were 13 randomized controlled trials. The experimental group was treated with electroacupuncture and the control group with western medicine. One is oral flutamine sustained release tablets, one is oral meloxicam, one is oral glucosamine hydrochloride, one is oral Fenbid, two are diclofenac sodium, three are ibuprofen, one is celecoxib, one is oral glucosamine hydrochloride, one is oral Fenbid, two are diclofenac sodium, three are ibuprofen, one is celecoxib. One was Zhuanggujianwan and the other was oral Fu Talin or Fenbid .Meta analysis showed that the efficacy and safety of electroacupuncture in treating knee osteoarthritis were statistically significant compared with the drug. Conclusion: electroacupuncture treatment of knee osteoarthritis is more effective and safe than that of drug group.
