补肾法对前交叉韧带重建后腱-骨愈合的影响

【摘要】：背景:前期研究现,使用补肾中药对兔前交叉韧带重建后的腱-骨愈合具有促进作用。目的:进一步验证补肾中药对前交叉韧带重建后腱-骨愈合的影响。方法:将70例因前交叉韧带损伤行前交叉韧带重建手术治疗的患者随机分为两组,观察组给予六味地黄丸及塞来昔布胶囊治疗,对照组给予塞来昔布胶囊治疗。分别对两组Lysholm客观评分、IKDC主观评分及MRI评估进行比较。结果与结论:(1)两组治疗后Lysholm评分均较治疗前提高(P0.05),观察组在不稳、爬楼项目以及总分均优于对照组(P0.05);(2)两组在治疗后IKDC评分均较治疗前明显提高(P0.05),观察组在打软腿、主观功能项目以及总分均优于对照组(P0.05);(3)治疗后6个月,观察组移植物中段、近端的扩大率均大于对照组(P0.05),治疗后12个月,观察组移植物远端、中段的扩大率均大于对照组(P0.05);(4)两组治疗后胫骨骨道以及股骨端隧道均有扩大(P0.05),观察组胫骨骨道扩大率小于对照组(P0.05);(5)治疗后第6个月,观察组在胫骨端、中段的信噪比均高于对照组(P0.01),治疗后12个月,观察组胫骨端、中段的信噪低于对照组(P0.05);(6)结果说明,补肾中药对前交叉韧带重建后腱-骨愈合具有促进作用,增强移植物在关节内的再生能力。
[Abstract]:Background: previous studies have shown that Chinese herbs for tonifying the kidney can promote tendon-bone healing after anterior cruciate ligament reconstruction in rabbits. Objective: to further verify the effect of kidney tonifying Chinese medicine on tendon-bone healing after anterior cruciate ligament reconstruction. Methods: seventy patients with anterior cruciate ligament injury were randomly divided into two groups. The observation group was treated with Liuwei Dihuang pill and celecoxib capsule, the control group was treated with celecoxib capsule. The objective score of Lysholm and the subjective score of IKDC and the evaluation of MRI were compared between the two groups. Results and conclusion: (1) after treatment, the Lysholm scores of the two groups were higher than those before treatment (P0.05). The observation group was superior to the control group in terms of instability, climbing items and total score (P0.05); (2). The IKDC score of the two groups after treatment was significantly higher than that before treatment (P0.05), and the observation group was beating soft legs. Subjective functional items and total scores were superior to those of the control group (P0.05); (3) 6 months after treatment. The enlargement rate of the middle segment and proximal end of the graft in the observation group was higher than that in the control group (P0.05), 12 months after treatment, the distal end of the graft in the observation group was higher than that in the control group. The enlargement rate of middle segment of tibia was higher than that of control group (P0.05); (4). The expansion rate of tibial bone canal and femur tunnel in observation group was lower than that in control group (P0.05); (5) 6 months after treatment (P0.05). The signal-to-noise ratio of middle segment was higher than that of control group (P0.01). Twelve months after treatment, the signal-noise of tibia end and middle segment of observation group was lower than that of control group (P0.05); (6). The results showed that Chinese herbs for tonifying kidney could promote tendin-bone union after anterior cruciate ligament reconstruction. Enhance the ability of graft regeneration in the joint.
