低频脉冲刺激穴位法治疗DPN疗效观察及下肢动脉状态对疗效影响

【摘要】：目的:通过治疗前后临床症状改善有效率的比较,研究综合治疗加用低频脉冲刺激穴位疗法是否可以提高DPN患者临床症状的改善程度,同时比较不同下肢动脉狭窄程度对临床治疗有效率的影响。材料与方法:将符合标准的60例患者分为两组进行试验。所有患者均进行基础糖尿病教育,同时均给予相同常规中西医结合降糖方案。治疗组在基础治疗上,加用低频脉冲治疗仪刺激穴位治疗,两组疗程均为12天。对患者治疗前后的临床症状情况进行调查评分,并在过程中观察患者有无不良反应,以判定低频脉冲治疗仪刺激穴位治疗的有效性,同时比较不同下肢动脉狭窄程度对患者临床症状治疗有效率的影响。结果:1治疗完成时,治疗组总有效率为86.67%,对照组总有效率为80%,经统计学比较,得出P0.05,说明差异具有统计学意义,说明低频脉冲刺激穴位疗法在改善DPN气血两虚兼血瘀证的临床症状方面优于对照组,提示联合该方法治疗DPN可明显提高临床治疗的有效率。2治疗结束后,两组共60例患者中,伴有下肢血管狭窄者共4例,治疗完成后治愈0例,显效0例,有效1例,无效3例,总有效率25%;伴有下肢动脉可疑狭窄者共23例,治疗完成后治愈1例,显效5例,有效12例,无效5例,总有效率78.26%;下肢血管无狭窄者33例,治疗完成后治愈2例,显效13例,有效17例,无效1例,总有效率96.97%;数据经过统计学处理后,P0.05,具有统计学意义,说明下肢动脉狭窄程度对DPN治疗的效果有一定的影响。结论:1.在中西医综合用药的基础上加用低频脉冲穴位治疗,可提高DPN患者临床症状的改善程度。2.DPN患者下肢动脉狭窄程度对临床疗效有影响,动脉狭窄程度越低,临床症状改善越明显。
[Abstract]:Objective: to compare the effective rate of clinical symptom improvement before and after treatment, and to study whether combined therapy with low frequency pulse stimulation can improve the clinical symptoms of DPN patients. At the same time, the effect of different degree of arterial stenosis of lower extremity on the effective rate of clinical treatment was compared. Materials and methods: 60 patients were divided into two groups. All patients received basic diabetes education and were given the same routine integrated traditional Chinese and western medicine hypoglycemic program. In the treatment group, the treatment group was treated with low frequency pulse therapy instrument and acupoint stimulation. The course of treatment in both groups was 12 days. The clinical symptoms of the patients before and after treatment were investigated and the adverse reactions were observed in order to determine the effectiveness of acupoint stimulation with low frequency pulse therapy instrument. At the same time, the effect of different degree of arterial stenosis of lower extremity on the effective rate of clinical symptom treatment was compared. Results the total effective rate of the treatment group was 86.67 and the total effective rate of the control group was 80 when the treatment was completed. The difference was statistically significant (P 0.05). The results showed that the low frequency pulse stimulation acupoint therapy was superior to the control group in improving the clinical symptoms of deficiency of both qi and blood stasis in DPN. It was suggested that the combined therapy with this method could significantly improve the effective rate of the treatment of DPN after the end of the treatment. There were 4 cases with vascular stenosis of lower extremity, 0 cases were cured after treatment, 0 cases were effective, 1 case was effective, 3 cases were ineffective, the total effective rate was 25 cases, 23 cases were accompanied with suspected stenosis of lower extremity artery, 1 case was cured after treatment, 5 cases showed remarkable effect. 12 cases were effective, 5 cases were ineffective, the total effective rate was 78.26, 33 cases had no stenosis in lower extremity, 2 cases were cured, 13 cases were effective, 17 cases were effective, 1 case was ineffective, and the total effective rate was 96.97%. It indicated that the degree of lower extremity artery stenosis had certain influence on the effect of DPN. Conclusion 1. The treatment of low frequency pulse acupoints on the basis of comprehensive Chinese and western medicine can improve the degree of improvement of clinical symptoms of DPN patients. 2. The degree of lower extremity artery stenosis in patients with DPN has an effect on the clinical curative effect. The lower the degree of arterial stenosis, the more obvious the improvement of clinical symptoms.
