清热化痰活血祛瘀法治疗痰瘀互阻型不稳定型心绞痛的临床研究

【摘要】：目的:本研究旨在观察清热化痰活血祛瘀法治疗痰瘀互阻型不稳定型心绞痛的临床疗效,并探究其作用机理。方法:本临床研究中,所选取的病例均来自2014月9月至2015年12月山东中医药大学附属医院综合内科病房的住院病人,共60例,所选病例均符合纳入排除标准,随机分为试验组与对照组,各30例,二组患者的一般情况无统计学差异。两组均给予西医常规综合治疗,试验组在此基础上给予清热化痰活血祛瘀方,日一剂,分早、晚两次温服。4周后观察治疗前后两组的中医症候积分、中医证候疗效、心电图变化、硝酸甘油停减率、血脂、血流变、纤维蛋白原等相关实验室检查,通过对比分析,评价其临床疗效。结果:试验组与对照组疗效相比,试验组在中医证候积分方面改善更加明显、改善心绞痛发作方面优于对照组,在硝酸甘油停减率、心电图变化、TC、TG等实验室指标方面,两组对比有统计学差异(P0.05或P0.01),试验组较对照组有更好的疗效,在肝肾功等安全性指标方面,二者无统计学差异,P0.05,应用安全。结论:相比单纯西医常规治疗,试验组在改善患者临床症状及相关实验室指标方面疗效更加的显著,且应用安全。因此,其在治疗痰瘀互阻型不稳定型心绞痛方面有着广阔的前景及优势。
[Abstract]:Objective: to observe the clinical effect of clearing heat, removing phlegm and activating blood circulation and removing blood stasis in the treatment of unstable angina pectoris of phlegm and blood stasis. Methods: in this clinical study, 60 inpatients were selected from the general medical wards of the affiliated Hospital of Shandong University of traditional Chinese Medicine from September, 2014 to December, 2015. All the selected cases met the exclusion criteria. The patients were randomly divided into the experimental group and the control group with 30 cases each. There was no statistical difference in the general situation between the two groups. Both groups were given routine comprehensive treatment of western medicine, and the experimental group was given the prescription of clearing heat, resolving phlegm and activating blood circulation and removing blood stasis on this basis, one dose a day, two times in the morning, two times in the evening, after 4 weeks, observe the integral of TCM symptoms and the curative effect of TCM syndrome before and after the treatment. The changes of electrocardiogram, stopping rate of nitroglycerin, blood lipid, hemorheology, fibrinogen and other related laboratory tests were compared and analyzed to evaluate the clinical effect. Results: compared with the control group, the improvement of TCM syndromes and angina pectoris in the trial group was better than that in the control group. There was statistical difference between the two groups (P0.05 or P0.01). The experimental group had better curative effect than the control group. There was no statistical difference between the two groups in terms of safety indexes such as liver and kidney function. Conclusion: compared with routine western medicine, the treatment group is more effective and safe in improving clinical symptoms and related laboratory indexes. Therefore, it has broad prospects and advantages in the treatment of unstable angina pectoris with phlegm and blood stasis.
