蛇床子散加减方治疗湿热下注型外阴上皮非瘤样病变的临床疗效观察

【摘要】：目的:观察蛇床子散加减方熏洗治疗湿热下注型外阴上皮非瘤样病变的临床疗效,通过本研究为采用蛇床子散加减方治疗外阴上皮非瘤样病变提供可靠的临床依据,并评估蛇床子散加减方治疗外阴上皮非瘤样病变的有效性以及安全性。方法:选取2015年1月至2017年1月于江苏省中医院妇科门诊就诊,符合纳入标准的外阴上皮非瘤样病变患者30例,予蛇床子散加减方熏洗治疗,按照自身治疗前后对照原则,评价蛇床子散加减方治疗外阴上皮非瘤样病变的临床疗效。依据本病的病变特点、中药外洗的治疗特点,并结合导师多年的临床经验,采用蛇床子散加减方每天1剂,熏洗外阴,14天为1个疗程。结果:患者症状总积分,治疗后与治疗前相比,差异有统计学意义(P0.05);治疗后患者瘙痒、皮肤弹性及颜色、病损范围积分与治疗前相比,均有明显改善,差异有统计学意义(P0.05),且以瘙痒症状改善最为显著;治疗后,各症状随着治疗时间的延长,均有明显改善,其中瘙痒症状在第一、二疗程改善最明显;治疗后皮肤弹性及颜色均在第三、四疗程改善最明显;治疗后病损面积改善在最后两个疗程改善最明显;不同病程患者之间的症状改善情况比较,差异无统计学意义(P0.05);中度患者治疗后症状改善程度优于轻度患者(P0.05);6个疗程后,30例患者中,痊愈7例,痊愈率23.33%;显效15例,显效率50.00%;好转6例,好转率20.00%;无效2例,无效率6.67%,无不良反应发生,总有效率为93.33%。结论:中药熏洗治疗外阴上皮非瘤样病变安全无创可靠,经过临床病例观察,蛇床子散加减方治疗外阴上皮非瘤样病变有很好的临床疗效,无不良反应及过敏反应发生,值得进一步推广应用于临床。
[Abstract]:Objective: to observe the clinical effect of Cnidium powder plus subtractive prescription fumigation in the treatment of non-tumor-like lesions of vulvar epithelium, and to provide a reliable clinical basis for the treatment of non-tumor-like lesions of vulvar epithelium by using Cnidium powder plus subtraction prescription. To evaluate the efficacy and safety of Cnidium Powder plus or minus prescription in the treatment of vulvar epithelial non-tumor-like lesions. Methods: from January 2015 to January 2017, 30 cases of vulvar epithelial nontumor-like lesions were selected from gynecological outpatient clinic of Jiangsu Provincial Hospital of traditional Chinese Medicine, treated by fumigation and washing of Cnidium Powder plus or minus prescription, and according to the principle of self-control before and after treatment. To evaluate the clinical effect of Cnidium Powder plus subtraction in the treatment of non-tumor-like lesions of vulvar epithelium. According to the pathological characteristics of the disease, the characteristics of external washing of traditional Chinese medicine and the clinical experience of teachers for many years, one dose of the prescription of adding and decreasing the powder of Cnidium cnidii was used to fumigate and wash the vulva for 14 days as a course of treatment. Results: the total score of symptoms after treatment was significantly higher than that before treatment (P0.05), and the patients' pruritus, skin elasticity and color, and the score of lesion range were significantly improved after treatment. The difference was statistically significant (P0.05), and the most significant improvement was the pruritus symptoms; after treatment, the symptoms were obviously improved with the prolongation of treatment time, among which, the pruritus symptoms were improved most obviously in the first and second courses of treatment. After treatment, the skin elasticity and color were significantly improved in the third and fourth courses of treatment; the improvement of lesion area was most obvious in the last two courses of treatment; the improvement of symptoms among patients with different course of disease was compared. The difference was not statistically significant (P0.05); the degree of improvement of symptoms after treatment in moderate patients was better than that in mild patients (P0.05); after 6 courses of treatment, 7 cases were cured, and the cure rate was 23.33%; 15 cases were markedly effective, the effective rate was 50.005%; 6 cases improved, the improvement rate was 20.00%; 2 cases were ineffective. The total effective rate was 93.33. Conclusion: fumigation and washing of traditional Chinese medicine is safe and noninvasive in the treatment of nontumor-like lesions of vulvar epithelium. The clinical observation shows that Cnidium nididii Powder plus subtractive prescription has a good clinical effect in the treatment of non-tumor-like lesions of vulvar epithelium without adverse reactions and allergic reactions. It is worth further popularizing and applying in clinic.
