黄连降糖片改善2型糖尿病胰岛素抵抗的临床研究

【摘要】：目的:本课题研究拟通过临床观察“黄连降糖片”治疗2型糖尿病胰岛素抵抗的临床疗效和安全性,初步评价“黄连降糖片”的作用机理及其安全性,为中医药改善胰岛素抵抗提供新的思路与方法。方法:本课题随机选取开封市中医院内分泌科门诊符合2型糖尿病(T2DM)胰岛素抵抗的诊断标准,符合中医湿热互结辨证标准的T2DM患者66例,探讨“黄连降糖片”改善糖尿病胰岛素抵抗的临床疗效。同时观察,受试者用黄连降糖片后的临床症状,空腹血糖(FPG),餐后2小时血糖(2h PG),糖化血红蛋白(Hb A1C),空腹胰岛素(FINS),胰岛素抵抗指数(HOMA-IR),总胆固醇(TC),甘油三脂(TG),高密度脂蛋白(HDL-C),低密度脂蛋白(LDL-C)等指标以及中医临床症状积分治疗前后变化,为临床用药提供科学依据。结果:1.治疗组的中医症候有效率优于对照组,差异有显著意义(P0.05);2.两组治疗后中医症状积分均减少,同组内治疗前后相比有统计学意义(P0.05);组间比较治疗组优于对照组,差异有统计学意义(P0.05)。3.两组均可降低FPG、2h PG、Hb A1C,同组内治疗前后相比有统计学意义(P0.05);治疗后组间FPG、2h PG比较,治疗组优于对照组,差异有统计学意义(P0.05)。治疗后两治疗后组间比较,Hb A1C无明显差异(P0.05)。4.两组均可降低FINS、HOMA-IR,同组内治疗前后相比有统计学意义(P0.05);组间比较治疗组优于对照组,差异有统计学意义(P0.05)。5.两组均能降低TC、TG、LDL-C,治疗组可升高HDL-C,差异有显著意义(P0.05)。6.两组试验过程中三大常规和肝肾功能等未出现异常。结论:“黄连降糖片”能够很好的改善患者的症状和体征,可以降低血糖、减轻胰岛素抵抗,还能够有效调节脂代谢紊乱,初步判定其降糖作用可能与改善胰岛素抵抗相关。
[Abstract]:Objective: to observe the clinical efficacy and safety of Huanglian Jiangtang tablet in the treatment of insulin resistance in type 2 diabetes mellitus, and to evaluate the mechanism and safety of Huanglian Jiangtang tablet. To provide new ideas and methods for traditional Chinese medicine to improve insulin resistance. Methods: this study randomly selected 66 T2DM patients who met the diagnostic criteria of insulin resistance of type 2 diabetes mellitus (T2DM) in the Department of Endocrinology, Kaifeng Hospital of traditional Chinese Medicine. To investigate the clinical effect of Huanglian Jiangtang tablet on improving insulin resistance of diabetes mellitus. At the same time, the clinical symptoms of Huanglian Jiangtang tablet, 2 hours postprandial blood glucose of (FPG), (2 h PG), glycosylated hemoglobin (Hb A1C), fasting insulin (FINS), insulin resistance index (HOMA-IR) were observed. The changes of total cholesterol (TC), triglyceride (TG), high density lipoprotein (HDL-C), low density lipoprotein (LDL-C) and clinical symptom integral of traditional Chinese medicine (TCM) before and after treatment provide scientific basis for clinical use of drugs. The result is 1: 1. The effective rate of TCM symptom in the treatment group was better than that in the control group, the difference was significant (P0.05); 2. The scores of TCM symptoms in the two groups were decreased after treatment, there was statistical significance before and after treatment in the same group (P0.05); the comparison between the treatment group and the control group was better than that in the control group, the difference was statistically significant (P0.05). Both groups can reduce FPG,2h PG,Hb A1C, the same group before and after treatment compared with the statistical significance (P0.05); after treatment, the treatment group was better than the control group, the difference was statistically significant (P0.05). There was no significant difference in Hb A1C between the two groups after treatment (P0.05). Both groups can reduce FINS,HOMA-IR, in the same group before and after treatment compared with statistical significance (P0.05), the treatment group was better than the control group, the difference was statistically significant (P0.05). Both groups can reduce the TC,TG,LDL-C, treatment group can increase HDL-C, there was significant difference (P0.05). The three major routine and liver and kidney function were not abnormal in the two groups. Conclusion: Huanglian Jiangtang tablet can improve the symptoms and signs of the patients, reduce blood sugar, reduce insulin resistance, and regulate lipid metabolism disorder effectively. It is preliminarily concluded that its hypoglycemic effect may be related to the improvement of insulin resistance.
