温针灸治疗女性尿道综合征的临床研究

【摘要】：女性尿道综合征一直是个相当敏感又尴尬的话题,主要表现症状为尿频、尿急、排尿不畅,另外女性尿道综合征还对女性的社会互动、性生活、生活质量等方面有着严重影响,甚至会使女性产生羞耻、抑郁等情况,从而导致患有女性尿道综合征女性出现身体形象的改变或者是社交隔离等现象。女性尿道综合征就像是一颗埋在女性身体和心里的不定时炸弹,较多女性其实很害怕在外面时,因为尿频、尿急,而不能及时找到找到厕所进行排解,因此也不敢轻易的外出,出门也会尽量选择有比较多厕所的地方,对部分人来说这样的情形,在生活上产生了较大的影响。目前在国内可查的文献当中了解到,尿道综合征在西方医学的治疗方面,多数是服用药物、手术治疗、介入治疗等,在中医治疗方面也很多元化,有中药治疗、针刺配合电针、艾灸治疗、埋线治疗、耳穴治疗等。目的：本课题采用临床小样本随机对照研究,通过观察温针灸组与常规针刺组治疗女性尿道综合征的患者。观察治疗前后症状体征的变化明确,希望透过此研究能取得较好的临床疗效。研究方法：将符合要求的56例患者随机分为观察组29例和对照组27例。观察组采取温针灸治疗,对照组采取针刺治疗,针刺穴位选取以腹部为主,每周2次,每10次为一个疗程。2个疗程之后进行观察评比。评分量表在治疗前及治疗结束后各评价1次,对比治疗前后的疗效及症状改善情况。结果：1、治疗前,试验组与对照组在患者年龄及病程等指标,经统计学检验,均无明显差异(P0.05),具有可比性。试验组与对照组在各症状评分分别进行统计学检验,结果无明显差异(P0.05),具有可比性。2、经过2个疗程的治疗,FUSS和QOL观察患者治疗后对于尿道综合症及生活质量都有改善,试验组有效率82.76%,对照组有效率77.78%,说明两组对尿道综合征均有治疗效果。SDS观察患者治疗后对于抑郁情况的方面也有改善。3、试验组和对照组在治疗后尿频、尿急、排尿不畅等症状,在指标的组内比较有统计学意义(P0.05),说明两组治疗方法对以上症状的改善有一定的疗效。结论：温针灸治疗与针刺治疗两种的治疗方式,对女性患者均能有效的改善尿道综合征,生活质量亦都有效改善。本研究经过两种的治疗观察,温针灸治疗效果优于常规针刺治疗。此方法为临床研究提供了新的思路方向。
[Abstract]:Female urethral syndrome has always been a very sensitive and embarrassing topic. Its main symptoms are frequency of urination, urgency of urination, unobstructed urination. In addition, female urethral syndrome also has a serious impact on women's social interaction, sexual life, quality of life, etc. It may even lead to shame, depression and other conditions, resulting in women with female urethral syndrome, such as changes in body image or social isolation. Female urethra syndrome is like an unfixed time bomb buried in a woman's body and heart. More women are actually very afraid to be outside because of frequent urination and urination, so they cannot find the toilet in time for drainage, so they do not dare to go out easily. Go out will also try to choose more toilet places, for some people such a situation, in life has a greater impact. At present, among the available literature in China, it has been learned that the treatment of urethral syndrome in western medicine is mostly drug taking, surgical treatment, interventional therapy, and so on. There are also many kinds of Chinese medicine treatment, including traditional Chinese medicine treatment, acupuncture combined with electroacupuncture, etc. Moxibustion therapy, catgut embedding therapy, ear point therapy, etc. Objective: to observe the effect of warming acupuncture group and routine acupuncture group on female urethral syndrome. Observing the changes of symptoms and signs before and after treatment, we hope to obtain better clinical effect through this study. Methods: 56 patients were randomly divided into observation group (n = 29) and control group (n = 27). The observation group was treated with warm acupuncture and moxibustion, the control group was treated with acupuncture, and the acupuncture points were mainly selected as abdomen, twice a week, every 10 times as a course of treatment. The score scale was evaluated once before and after treatment, and the curative effect and symptom improvement before and after treatment were compared. Results: 1, before treatment, there was no significant difference between the test group and the control group in patients' age and course of disease (P0.05). There was no significant difference between the test group and the control group in each symptom score (P0.05). 2, after two courses of treatment, there was no significant difference between the test group and the control group. FUSS and QOL were used to observe the improvement of urethral syndrome and quality of life after treatment. The effective rate was 82.76 in the experimental group and 77.78 in the control group. The results showed that the two groups were effective in treating urethral syndrome. After SDS treatment, the depression was also improved. 3. The symptoms of frequency of urination, urgency of urination and dysuria after treatment were observed in the test group and control group. In the index of the group compared with statistical significance (P0.05), indicating that the two groups of treatment methods to improve the above symptoms have a certain effect. Conclusion: both warming acupuncture and acupuncture treatment can effectively improve the urethral syndrome and the quality of life in female patients. After two kinds of treatment observation, the effect of warming acupuncture is better than that of routine acupuncture. This method provides a new way of thinking for clinical research.
