双侧针刺健侧通电治疗原发性三叉神经痛的临床观察

【摘要】：目的:通过观察双侧针刺健侧通电治疗原发性三叉神经痛与口服卡马西平治疗疗效差异,为治疗原发性三叉神经痛提供一种疗效确切,操作简单,经济安全的治疗方法。方法:将62例来自黑龙江中医药大学附属第二医院针灸五门诊的符合纳入标准的原发性三叉神经痛患者按照随机分组法分成两组,即治疗组(针刺组)和对照组(西药组),每组各31例。对照组(西药组)口服卡马西平(由北京曙光药业有限责任公司生产,规格为0.1g/片)。首剂0.1g,每日2次。以后每天增加0.1g,直到疼痛停止,但是最大量不能超过1000mg/d。治疗组(针刺组):患者仰卧位,或坐立位。百会、予针刺后施以强捻转快速法,持续约2分钟。面部腧穴采取常规透刺手法,手法柔和,得气即止。远端腧穴采取常规针刺手法,平补平泻。用英迪牌KWD808Ⅱ电针治疗仪,密波70HZ、波形固定,根据患者的耐受程度可做适当的加减,导线连接:连接健侧太阳(+),四白(-);连接健侧:下关(+),夹承浆(-)。每次治疗持续30分钟,每日针刺1次,6天为一疗程,共四个疗程,每个疗程间隔一天。于治疗前,治疗后分别详细采集记录患者视觉模拟评分(VAS)、生活质量评分、疼痛症状积分进行评价。结果:1.本试验共纳入62例患者,过程中治疗组和对照组各有一例脱落,最终治疗组对照组各有30例患者完成试验。2.疗效评价:经课题设计的方法治疗完成过后,治疗组(针刺组)治愈5例,显效17例,有效5例,无效3例;对照组(西药组)治愈2例,显效11例,有效12例,无效5例。治疗组总有效率90.00%,对照组总有效率83.33%。经统计学分析,试验结果具有统计学意义(P0.05),可得出治疗组疗效优于对照组疗效的结论。3.VAS评分、生活质量评分以及疼痛症状积分,治疗组和对照组的这三者在自身治疗前后的对照,均有显著差异(P0.01),且治疗组优于对照组(p0.05),差异有统计学意义。结论:1.治疗组、对照组对治疗原发性三叉神经痛均有效,治疗组疗效优于对照组。2.治疗组、对照组对于降低VAS评分、疼痛症状积分、生活质量评分均有效,治疗组效果优于对照组。即是治疗组在减轻PTN患者的临床症状和痛苦以及提高生活质量上优于对照组。3.双侧针刺健侧通电是一种有效且操作简便的治疗方式,值得进一步的研究和推广。
[Abstract]:Objective: to observe the difference of therapeutic effect of bilateral acupuncture on primary trigeminal neuralgia and oral carbamazepine in the treatment of primary trigeminal neuralgia, and to provide an effective, simple, economical and safe method for the treatment of primary trigeminal neuralgia.
