黄白合剂治疗肛裂术后创面愈合临床观察

【摘要】:目的:探讨湿热下注型肛裂患者术后,使用黄白合剂坐浴熏洗外用治疗,了解黄白合剂对创口疼痛、创面分泌物的PH值、创面面积及创面愈合情况等方面的影响。方法:按照随机数表法,将符合纳入标准的120例患者,分为治疗组和对照组各60例。两组在手术前后采用相同的常规治疗基础上,治疗组于术后第2天予院内制剂黄白合剂坐浴熏洗,每日1剂,早晚各坐浴1次,连续使用7天;对照组予高锰酸钾溶液熏洗坐浴。观察两组患者术后第2、7、14天创口疼痛、创面分泌物的PH值等情况进行评分并记录数据。在术后第7、14、21天记录创面愈合面积,比较创面愈合时间以及术后第21天两组的总疗效。结果:入选的120例患者,收集时无病例脱落。治疗前将两组病例的性别、年龄、病程、创面疼痛、创面分泌物的PH值等进行比较,差异均无统计学意义,具有可比性。研究结果分析如下:(1)创面疼痛:两组术后第7、14天创面疼痛情况比较均呈显著性差异(P0.05),黄白合剂组明显优于高锰酸钾组,两组术后第2天治疗前创面疼痛比较均无显著性差异(P0.05)。(2)创面分泌物的PH值:两组术后第7、14天创面疼痛情况比较均呈显著性差异(P0.05),治疗组明显优于对照组,两组术后第2天治疗前创面疼痛比较均无显著性差异(P0.05)。(3)创面面积大小、创面愈合时间和总疗效:两组术后第7、14、21天创面的面积情况对比存在差异(P0.05),治疗组优于对照组。治疗组平均创面愈合时间(14.65±1.298)天,对照组平均创面愈合时间(17.50±1.374)天,创面愈合时间对比存在显著性差异(P0.05),治疗组优于对照组。术后第21天,治疗组总疗效为100%,对照组总疗效为95%,两者对比无显著性差异(PO.05)。结论:黄白合剂可以缓解湿热下注型肛裂患者术后创面疼痛,.可能与肛门局部抑菌消炎抗感染有关;黄白合剂可以降低创面分泌物的PH值,可能与微酸环境更益于细胞的生长有关;黄白合剂可以缩短创面上皮化的时间,可能与促进创面的肉芽组织生长有关;黄白合剂可以改善创面局部血供,调节机体免疫实现并加强组织的修复机能,进而促进创面愈合。且用药过程中未出现不良反应,是一种有效安全的中药汤剂,临床值得推广。
[Abstract]:Objective: to investigate the effect of Huangbai mixture on wound pain, PH value of wound secretion, wound area and wound healing. Methods: according to the method of random number, 120 patients were divided into treatment group (n = 60) and control group (n = 60). The two groups were treated with the same routine treatment before and after operation. The treatment group was treated with Huangbai mixture in the hospital on the second day after operation, one dose daily, one sitting bath in the morning and evening for 7 days. The control group was treated with potassium permanganate solution fumigation and sitting bath. The wound pain and PH value of wound secretion were evaluated and recorded on the 2nd day and 14th day after operation in both groups. The wound healing area was recorded on day 7, 14 and 21 after operation, and the time of wound healing and the total curative effect of the two groups were compared. Results: none of the 120 patients were abscission at the time of collection. Before treatment, the sex, age, course of disease, wound pain, PH value of wound secretion of the two groups were compared, the difference was not statistically significant and comparable. The results were as follows: (1) wound pain: there was significant difference between the two groups on the 14th day after operation (P0.05), Huangbai mixture group was significantly better than potassium permanganate group. There was no significant difference in wound pain between the two groups before treatment on the second day after operation (P0.05). (2). The PH value of wound secretion was significantly different between the two groups on the 7th and 14th day after operation (P0.05), and there was no significant difference between the two groups before treatment (P0.05). The treatment group was obviously superior to the control group. There was no significant difference in wound pain between the two groups before treatment on the second day after operation (P0.05). (3), and there was no significant difference in wound area between the two groups before treatment (P0.05). Wound healing time and total curative effect: there were differences in the area of wound between the two groups on the 7th day and the 14th day after operation (P0.05), the treatment group was better than the control group. The average wound healing time was (14.65 卤1.298) days in the treatment group and (17.50 卤1.374) days in the control group. There was significant difference in the wound healing time between the treatment group and the control group (P0.05). On the 21st day after operation, the total curative effect of the treatment group was 100 and that of the control group was 95. There was no significant difference between the two groups (PO.05). Conclusion: Huangbai mixture can relieve postoperative wound pain in patients with anal fissure caused by dampness and heat. Huangbai mixture can reduce the PH value of wound secretion and may be related to the microacid environment which is more beneficial to cell growth. Huangbai mixture can shorten the time of wound epithelization, which may be related to promoting the growth of granulation tissue, and Huangbai mixture can improve the local blood supply of wound, regulate the realization of body immunity and strengthen the repair function of tissue, and then promote wound healing. It is an effective and safe traditional Chinese medicine decoction, which is worth popularizing.
