IL-4等炎症因子与登革热中医证型的相关性研究

【摘要】:目的:本课题将普通登革热患者急性期血清中6种细胞因子和中医的证型相结合,观察不同中医证型细胞因子的表达水平,从中研究两者是否存在关联性,使西医的诊断和微观指标与中医的辨证论治相结合,形成登革热独特的中西医结合诊疗规律,为其辨证分型提供客观依据,并筛选出有提示意义的指标,有助于提高中医药在登革热的诊治和防控领域的应用,使登革热在中医临床上的辨证分型和诊治更加可靠和准确。方法:利用广州中医药大学第一附属医院的电子病历系统,对登革热住院病人的基本资料和中医证型进行检索搜集,从中随机选取卫气同病型患者41例(A组)、湿热遏阻型患者41例(B组)和20例健康者(C组)。实验采用免疫学酶联免疫吸附法(ELISA方法)检测各组样本血清中炎症细胞因子IL-4、IL-12、IL-17、IFN-γ、TNF-α和TGF-β1的水平,将登革热中医的卫气同病证、湿热遏阻证和健康对照组的结果进行统计学分析。结果:(1)IL-4在湿热遏阻组与健康对照组之间有差异,P=0.0430.05,且湿热遏阻组高于健康对照组。(2)IL-12在卫气同病组与健康对照组之间有显著性差异,P=0.0000.01,且卫气同病组高于健康对照组;IL-12在湿热遏阻组与健康对照组之间有显著性差异,P=0.0000.01,且湿热遏阻组高于健康对照组。(3)IFN-γ在卫气同病组与健康对照组之间有显著性差异,P=-0.0050.01,且卫气同病组高于健康对照组;IFN-γ在湿热遏阻组与健康对照组之间有显著性差异,P=0.0380.05,且湿热遏阻组高于健康对照组。(4)登革热组内IL-4与IL-12、IL-17、IFN-γ都存在显著性正相关(f0.01),IL-4与TGF-β1存在正相关(P0.05);IL-12与IL-17、IFN-γ存在显著正相关(P0.01); IL-17与IFN-γ存在显著正相关(P0.01)。结论:(1)登革热卫气同病证患者血清IL-12和IFN-γ比健康对照组明显升高。(2)登革热湿热遏阻证患者血清IL-4、IL-12和IFN-γ比健康对照组明显升高。(3)登革热患者存在着炎症细胞因子的过度激活,并与中医证型密切相关,提示炎症细胞因子可作为登革热中医辨证分型的重要客观化指标。(4)登革热细胞因子之间的相关性分析发现,炎症细胞因子的释放在炎症反应过程中存在相互促进的作用。
[Abstract]:Objective: to observe the expression level of cytokines in different TCM syndromes by combining six cytokines in the serum of patients with dengue fever in the acute phase, and to investigate whether there is a correlation between the cytokines of different TCM syndromes. To combine the diagnosis and micro index of western medicine with the differentiation and treatment of traditional Chinese medicine, to form the unique law of diagnosis and treatment of dengue fever, to provide objective basis for the differentiation and classification of symptoms and signs, and to screen out the indicators of significance for the diagnosis and treatment of dengue fever. It is helpful to improve the application of traditional Chinese medicine in the field of diagnosis and control of dengue fever and to make the clinical diagnosis and treatment of dengue fever more reliable and accurate. Methods: using the electronic medical record system of the first affiliated Hospital of Guangzhou University of traditional Chinese Medicine, the basic data and TCM syndromes of inpatients with dengue fever were collected. 41 patients with the same disease of Wei Qi were randomly selected from the patients (group A). There were 41 cases of dampness and heat suppression type (group B) and 20 cases of healthy persons (group C). The levels of inflammatory cytokines IL-4,IL-12,IL-17,IFN- 纬, TNF- 伪 and TGF- 尾 1 in serum of each group were detected by immunological enzyme linked immunosorbent assay (ELISA method). The results of dampness and heat suppression syndrome and healthy control group were statistically analyzed. Results: (1) there was significant difference in IL-4 between damp-heat suppression group and healthy control group (P < 0.05), and the IL-12 of dampness and heat suppression group was higher than that of healthy control group. (2) there was a significant difference in IL-12 between Weiqi group and healthy control group. P0. 000 0. 01, and Wei Qi with disease group is higher than the healthy control group; There was a significant difference in IL-12 between damp-heat suppression group and healthy control group (P0. 0000. 01). (3) there was a significant difference in IFN- 纬 between Weiqi group and healthy control group, and the IFN- 纬 level of dampness and heat suppression group was higher than that of healthy control group. The patients in the same disease group were higher than those in the healthy control group. There was a significant difference in IFN- 纬 between damp-heat suppression group and healthy control group, which was 0.038 0.05, and higher in damp-heat suppression group than in healthy control group. (4) IL-4 and IL-12,IL-17, in dengue fever group were significantly higher than those in healthy control group. There was a positive correlation between IFN- 纬 and TGF- 尾 1 (f0.01), and a positive correlation between IL-4 and TGF- 尾 1 (P0.05). There was a significant positive correlation between IL-12 and IL-17,IFN- 纬 (P0.01), and between IL-17 and IFN- 纬 (P0.01). Conclusion: (1) the serum IL-12 and IFN- 纬 of the patients with the same syndrome of dengue qi and qi were significantly higher than those of the healthy control group. (2) the serum IL-4, of the patients with dampness and heat suppression syndrome of dengue fever was significantly increased. IL-12 and IFN- 纬 were significantly higher than those in healthy controls. (3) there was excessive activation of inflammatory cytokines in patients with dengue fever, which was closely related to TCM syndromes. The results suggest that inflammatory cytokines can be used as an important objective index in the differentiation and classification of dengue fever syndrome. (4) the correlation analysis between the cytokines of dengue fever revealed that the release of inflammatory cytokines played a mutually reinforcing role in the process of inflammation.
