膝关节肢段封闭式熏洗仪的研发及其在老年食蟹猴退变KOA的实验应用及研究

【摘要】:目的:膝关节骨性关节炎(KOA)是最常见的退行性骨关节疾病,熏洗疗法在KOA的治疗中越来越受到关注,但传统熏洗法存在空间开放,时间、温度不可控等缺点,针对以上问题,我科研发膝关节肢段封闭式熏洗仪,并使其联合独活寄生汤熏洗治疗自然退变KOA的老年食蟹猴,检测其对老年食蟹猴KOA关节软骨中Notch1、MMP-13、COL-Ⅱ水平表达的影响,探讨膝关节肢段封闭式熏洗仪对KOA的治疗效果,以及独活寄生汤熏洗治疗KOA的治疗效果及作用机制。方法:第一部分,进行膝关节肢段封闭式熏洗仪的设计与研发,并生产出样机,申请专利。第二部分,选取自然退变患有KOA的老年食蟹猴12只,随机分为实验组、对照组、空白对照组,每组各4只。实验组用膝关节肢段封闭式熏洗联合独活寄生汤熏洗,对照组运用传统熏洗法联合独活寄生汤熏洗,空白组用生理盐水进行传统熏洗处理。干预8周后处死动物,制作老年食蟹猴膝关节退变软骨细胞病理切片,镜下观察其病变后行mankin评分,最后对退变软骨细胞Notch1、MMP-13、COL-Ⅱ免疫组化染色,观察其表达。结果:(1)在继承传统熏洗法的基础上结合现代最新的生物科学技术完成膝关节肢段式熏洗仪的初步设计以及实体模型的制造,已生产出一台样机,申报了国家专利,专利号(ZL 2013 2 0763945.X)。(2)HE染色观察中,实验组、对照组Mankin评分均值低于空白对照组,组间比较差异具有统计学意义(P0.05),实验组Mankin评分均值低于对照组,组间比较差异具有统计学意义(P0.05)。在免疫组化观察中,实验组、对照组Notch1、MMP-13的表达均较空白对照组明显降低,组间比较差异有统计学意义(P0.05);实验组Notch1、MMP-13的表达率较对照组显著降低,组间比较差异有统计学意义(P0.05);实验组、对照组COL-Ⅱ的阳性表达率均较空白对照组明显升高,组间比较差异有统计学意义(P0.05);实验组COL-Ⅱ阳性表达率较对照组显著升高,组间比较差异有统计学意义(P0.05)。结论:(1)膝关节肢段封闭式熏洗仪较传统熏洗具有恒温、封闭、时间可控等优点;(2)独活寄生汤加减外用熏洗能调节膝关节软骨中Notch1、MMP-13、COL-Ⅱ的水平,抑制关节软骨的破坏,有效延缓膝关节退变,达到其治疗作用,膝关节肢段封闭式熏洗仪较传统熏洗法对KOA具有更好的治疗效果。
