针刺治疗血瘀型卵巢巧克力囊肿的临床观察

【摘要】：目的:对符合标准的血瘀型卵巢巧克力囊肿患者进行针刺治疗,观察针刺治疗对血瘀型卵巢巧克力囊肿患者的中医症候积分、CA125水平、囊肿最大径线及生存质量评分改善的情况。方法:将收集于黑龙江中医药大学附属第一医院妇科门诊符合血瘀型诊断标准的60例卵巢巧克力囊肿患者给予针刺治疗,根据针刺部位不同分成治疗组和对照组各30例。治疗组针刺关元、中极、地机、气海、子宫、天枢、次毼、血海(双侧)、三阴交(双侧)、足三里(双侧)、太冲(双侧)。对照组针刺非经络非穴位的位置,每日针刺1次,每次30分钟,1个月为一个疗程,连续治疗3个疗程后观察两组患者在治疗前后的中医症候积分、CA125水平、囊肿最大径线及生存质量方面的变化情况,综合评估针刺疗法治疗血瘀型卵巢巧克力囊肿的临床疗效。结果:1.治疗后两组的临床综合疗效评定结果显示治疗组痊愈1例,显效14例,有效10例,无效5例,总有效率高达83.33%,对照组痊愈0例,显效4例,有效8例,无效17例,总有效率为41.37%。治疗组优于对照组,疗效差异具有统计学意义(P0.05)。2.治疗后两组的中医症候积分相比差异显著,具有统计学意义(P0.05)。3.治疗后治疗组与对照组相比CA125水平明显下降,差异明显,具有统计学意义。4.治疗后治疗组与对照组相比巧囊最大径线明显缩小,差异具有统计学意义(P0.05)。5.治疗后两组的生存质量评分差异显著,具有统计学意义(P0.05)。结论:针刺疗法对血瘀型卵巢巧克力囊肿的临床治疗效果确切,可以提高临床综合疗效,显著改善中医症候积分,降低CA125数值,缩小病灶包块,提高患者的生存质量,提升包括控制疼痛的各项能力。
[Abstract]:Objective: to observe the TCM symptom integral and CA125 level of the patients with blood stasis type ovarian chocolate cyst treated by acupuncture, and to observe the effect of acupuncture treatment on the syndrome score and the level of TCM of the patients with blood stasis type ovarian chocolate cyst. The maximum diameter of the cyst and the improvement of the quality of life score. Methods: 60 patients with ovarian chocolate cyst were treated with acupuncture and divided into two groups: treatment group (n = 30) and control group (n = 30), who were collected from the Gynecology Clinic of the first affiliated Hospital of Heilongjiang University of traditional Chinese Medicine. After treatment, the clinical comprehensive curative effect evaluation of the two groups showed that the treatment group was cured in 1 case, remarkable effect in 14 cases, effective in 10 cases, ineffective in 5 cases, the total effective rate was 83.33%, the control group was cured in 0 cases, remarkable effect in 4 cases, effective in 8 cases, ineffective in 17 cases, the total effective rate was 83.33% in the control group. The total effective rate was 41.37%. The therapeutic effect of the treatment group was better than that of the control group, the difference was statistically significant (P0.05). 2. After treatment, the scores of TCM symptoms between the two groups were significantly different, with statistical significance (P0.05). 3. After treatment, the level of CA125 in the treatment group was significantly lower than that in the control group (P < 0.05). After treatment, the maximum diameter of the capsule in the treatment group was significantly smaller than that in the control group (P0.05). After treatment, there was significant difference in the quality of life score between the two groups (P0.05). Conclusion: acupuncture therapy is effective in the treatment of blood stasis ovarian chocolate cyst. It can improve the clinical comprehensive curative effect, significantly improve the TCM syndrome score, decrease the CA125 value, reduce the mass of the lesion, and improve the quality of life of the patients, and improve the quality of life of the patients.
