八味养脉汤联合四妙勇安汤对手足综合征的临床疗效观察

【摘要】：目的:本课题旨在通过以益气养血、活血通络法组成的八味养脉汤联合四妙勇安汤对手足综合征的临床疗效观察,从而探讨其对手足综合征可能的作用机制。方法:纳入2016年01月到2017年01月黑龙江中医药大学附属第一医院门诊和住院部符合纳入标准的气虚血瘀型手足综合征患者60例,采用随机对照的方法将其分成两组,试验组给予自拟中药口服配合经典中药汤剂外洗,对照组则给予维生素B6片口服配合温水外洗,21天后评价患者治疗前后手足综合征的分级变化、疼痛程度的分级变化、生活质量(卡氏)评分变化及治疗前后患者中医证候评分的变化。结果:1.在手足综合征分级的改善方面,试验组总有效率为80%,对照组的总有效率为55.17%,两组差异具有统计学意义(p0.05)。2.在疼痛程度的改善方面,试验组总缓解率是86.36%,对照组的总缓解率是52.63%,差异有统计学意义(p0.01)。3.在生活质量(卡氏)评分的改善方面,试验组的升高率为53.33%、稳定率为36.67%;对照组升高率为24.14%、稳定率为48.28%差异具有统计学意义(p0.05)。4.在中医证候积分改善方面,试验组的总有效率是86.67%,对照组总有效率是62.07%,两组差异有统计学意义(p0.05)。结论:1.八味养脉汤联合四妙勇安汤可以有效降低手足综合征分级;2.八味养脉汤联合四妙勇安汤可以明显降低手足综合征疼痛分级;3.八味养脉汤联合四妙勇安汤能够提高患者生活质量(卡氏)评分;4.八味养脉汤联合四妙勇安汤能够缓解气虚血瘀型手足综合征患者的症状,降低中医证候积分。
[Abstract]:Objective: the purpose of this study was to observe the clinical curative effect of Bawei Yangmai decoction combined with Simiao Yong'an decoction, which was composed of Yiqi Yangxue and Huoxue Tongluo method, so as to explore the possible mechanism of hand foot syndrome. Methods: from January 2016 to January 2017, 60 patients with hand and foot syndrome of Qi deficiency and blood stasis type were randomly divided into two groups in the outpatient department and inpatient department of the first affiliated Hospital of Heilongjiang University of traditional Chinese Medicine. The experimental group was given oral washing of self-made traditional Chinese medicine combined with classical traditional Chinese medicine decoction, while the control group was given vitamin B6 tablets combined with warm water external washing. 21 days later, the grading changes of hand and foot syndrome and the degree of pain were evaluated. The change of quality of life (Karschner) score and the change of TCM syndrome score before and after treatment. Result: 1. In the improvement of the classification of hand and foot syndrome, the total effective rate was 80% in the experimental group and 58.17% in the control group. There was significant difference between the two groups (p0.05). In terms of improving the degree of pain, the total remission rate was 86.36% in the experimental group and 52.63% in the control group, the difference was statistically significant (p0.01). In terms of the improvement of quality of life (Karst) score, the increase rate was 53.33% and the stability rate was 36.67% in the experimental group and 24.14% in the control group, and the stability rate was 48.28%, the difference was statistically significant (p0.05). 4. In the improvement of TCM syndrome score, the total effective rate was 86.67% in the experimental group and 62.07% in the control group. There was significant difference between the two groups (p0.05). Conclusion: 1. Bawei Yangmai decoction combined with Simiao Yong an decoction can effectively reduce the grade of hand and foot syndrome. 2. Bawei Yangmai decoction combined with Simiao Yong an decoction can significantly reduce the pain grade of hand and foot syndrome. 3. Bawei Yangmai decoction combined with Simiao Yong an decoction can improve the quality of life (Karst) score of patients. 4. Bawei Yangmai decoction combined with Simiao Yong'an decoction can relieve the symptoms of hand and foot syndrome with qi deficiency and blood stasis, and reduce the syndrome score of traditional Chinese medicine.
