自制温阳灸条治疗早期糖尿病肾病阳虚质的临床研究

【摘要】：目的:观察和研究自制温阳灸条治疗早期糖尿病肾病阳虚质的临床疗效及其对早期糖尿病肾病阳虚质患者血肌酐、血尿素氮、尿微量白蛋白、尿白蛋白排泄率、血糖、糖化血红蛋白等临床指标的影响,同时,观察自制温阳灸条对中医体质调节的影响,进而探讨自制温阳灸条对早期糖尿病肾病阳虚质治疗的影响机制,为中医外法治疗早期糖尿病肾病阳虚质提供理论依据。方法:选取70例早期糖尿病肾病阳虚质患者,采用单盲法,随机分为治疗组35例,对照组35例。对照组采用一般性治疗和常规对症治疗,再施以清艾条行温和灸法,治疗组在对照组治疗基础上换用自制温阳灸条行温和灸法。自制温阳灸条药物组成:将清艾条中艾绒与肉桂、黄芪、附子研成的细粉以2:1:1:1的比例混匀制成自制温阳灸条,规格为直径14mm、长度20mm。(中药、清艾条均来源于云南中医学院第一附属医院中药房)灸双侧肾俞穴、命门穴、关元穴,双侧足三里穴,施灸时将艾条一端点燃,插入艾灸架中约距皮肤2-3cm左右,使灸架对准应灸腧穴。每穴每次15 min。每日1次,每星期6次,2星期为1个疗程,共治疗3个疗程。治疗3个疗程后,观察两组治疗前后血肌酐、血尿素氮、24小时尿蛋白定量、尿微量白蛋白、尿白蛋白排泄率、血糖、糖化血红蛋白变化情况(安全性观测、诊断学疗效,中医临床症候疗效、中医体质调节情况)。结果:1.70例患者,10例被剔除,60例纳入本研究,患者依从性大于85.71%,两组患者在性别、年龄、病程、治疗前中医症候评分及中医体质等临床一般资料方面无显著性差异。2.经过3个疗程治疗后,治疗组总有效率为83.33%;对照组总有效率为60.00%,两组比较有显著性差异(P㩳0.05)。3.两组治疗后,血肌酐、血尿素氮、24小时尿蛋白定量、尿微量白蛋白、尿白蛋白排泄率、血糖、糖化血红蛋白与本组治疗前比较差异均有统计学意义(P0.05,P0.01);治疗组在降低血肌酐、24小时尿蛋白定量、尿微量白蛋白、尿白蛋白排泄率、血糖、糖化血红蛋白方面以及改善中医临床证候及阳虚质程度方面优于对照组(P0.05,P0.01)。结论:自制温阳灸条对早期糖尿病肾病阳虚质有较好的临床疗效,在提高整体疗效,改善患者中医临床症状,降低血肌酐、24小时尿蛋白定量、尿微量白蛋白、尿白蛋白排泄率、血糖、糖化血红蛋白方面,治疗组效果优于对照组。
