离体猪肝胆道内射频消融时局部胆汁温度及胆道损伤情况

【摘要】:目的探讨不同功率胆道内射频消融后离体猪肝局部胆汁温度变化及胆道损伤情况。方法选取10个新鲜离体猪肝(胆囊及胆总管中上段完整保留),于DSA下将收集的人胆汁注入猪胆道系统,并注入少量对比剂,然后将HabibTMEndoHPB射频消融导管经胆总管引入肝门区胆管内并用缝线将胆总管与消融导管绑紧。10个猪肝消融参数分别为5 W(120s)、6 W(120s)、7 W(120s)、8 W(120s)、9 W(120s)、10 W(120s)、11 W(120s)、12 W(120s)、13 W(120s)、14 W(120s)。透视下分别于远端消融电极、距远端消融电极1、2cm处插入测温针进行测温。消融后取消融区胆管及毗邻组织送病理检查评价损伤程度。结果 10个人胆汁充盈离体猪肝胆道系统消融模型均成功建立,且成功导入射频消融导管进行消融。测温显示消融区温度随功率增加而升高、最高达90.3℃;距远端消融电极1、2cm处温度上升不明显,为28.4~40.2℃。病理检查显示消融区胆管出现全层凝固性坏死,距消融区1、2cm处胆管及毗邻肝组织无明显损伤。结论胆道内消融可使邻近胆道内胆汁温度有所升高,但对毗邻非消融区胆管内皮无明显损伤,提示胆汁热传导作用对毗邻非消融区胆管及肝组织影响较小。
