轮椅振动训练对中年男性下肢功能障碍者平衡功能的影响

【摘要】:随着人类医学的发展,重大疾病所带来的死亡率逐年降低,但部分因疾病所带来的病发症却无法避免。因病导致下肢运动障碍者逐渐增多,由于身体原因,下肢运动功能障碍者无法自由活动,大多依靠轮椅代步,长时间坐于轮椅导致下肢肌力逐渐退化,甚至无法长时间行走,严重影响患者生活质量,为家庭及社会带来沉重负担。改善平衡功能是帮助他们完成日常生活基本动作的重要保障,也是患者进行日常活动的基础。振动训练作为一种新兴的康复训练方式,对于快速反复刺激肌肉,改善神经传导速率,增强肌肉活性,增加肌肉力量,提高平衡功能具有显著性的效果。本文将采用实验法,使用BODY GREEN轮椅全身垂直振动训练仪进行轮椅振动训练,探讨轮椅振动训练对下肢运动功能障碍者的平衡功能影响。本文旨在探寻更为有效、安全、便捷的康复训练方法,为改善下肢运动功能障碍者平衡功能提供理论依据,具有一定的理论意义和研究价值。研究方法:依据本研究所要求的纳入排除标准,于2015年9月~2015年12月,通过上海市杨浦区残障人联合会,招募20名下肢运动功能障碍的中年男性作为实验对象,进行为期8周的轮椅振动训练干预。在轮椅振动训练前后,每一位被试均使用CON-TREX等速测试仪、WIN-POD静态平衡仪检测,同时进TUG、10MWT和6MWT测试,所有测试均由相同工作人员完成,训练结束后进行自身前后对照,对题数据进行研究分析。研究结果:通过8周的轮椅振动训练,被试的膝、踝关节屈、伸肌群均有大幅度提高,膝关节伸肌峰力矩在60°/s和180°/s时具有显著性提高(P0.05),膝关节屈伸肌峰力矩比值也具有明显差异(P0.05)。踝关节与膝关节较为一致,踝关节伸肌群显著性略高于踝关节屈肌群,踝关节伸肌峰力矩有差异具有显著性(P0.05),而踝关节在屈伸肌峰力矩比值均无差异具有显著性。静态平衡在睁眼、闭眼双足站立结果均具有良好的变化,闭眼双足站立指标部分有差异具有显著性(P0.05);在TUGT和6MWT的测试结果为差异具有非常显著性(P0.01),10MWT测试结果差异也存在显著性(P0.05)。研究结论:8周的轮椅振动训练能够提高被试膝、踝关节肌力,说明轮椅振动训练可以作为下肢运动功能障碍者一个安全可行训练手段。由于轮椅振动训练对各个关节肌群作用效果并不是一致的,所以还有待进一步研究。轮椅振动训练可以显著性提高闭眼双足站立的测试结果,表明轮椅振动训练能够提高本体感觉以及前庭功能,并有效改善下肢运动功能障碍者身体控制能力。动态平衡较实验前后也有较大改善,由此可知,轮椅振动训练对中年下肢运动功能障碍者平衡功能有积极影响。因此我们认为轮椅振动训练可用于下肢运动功能障碍者的常规康复训练。
[Abstract]:With the development of human medicine, the mortality rate caused by major diseases is decreasing year by year, but some diseases caused by diseases can not be avoided. People with lower extremity motor dysfunction can not move freely, most of them rely on wheelchairs to walk. Sitting in wheelchairs for a long time leads to the deterioration of lower limb muscle strength and even the inability to walk for a long time, which seriously affects the quality of life of patients. The improvement of balance function is an important guarantee to help them complete the basic actions of daily life, and is also the basis for patients to carry out daily activities. Vibration training is a new rehabilitation training method. It has significant effect on fast and repeated stimulation of muscle, improving nerve conduction rate, enhancing muscle activity, increasing muscle strength and improving balance function. The purpose of this paper is to explore the effect of wheelchair vibration training on the balance function of patients with lower extremity motor dysfunction by using BODY GREEN wheelchair whole body vertical vibration training instrument to explore a more effective, safe and convenient rehabilitation training method. In order to provide theoretical basis for improving the balance function of patients with lower extremity motor dysfunction, it has certain theoretical significance and research value. Research methods: according to the exclusion criteria required by this study, from September 2015 to December 2015, Through Shanghai Yangpu District disabled people's Federation, 20 middle-aged men with lower extremity motor dysfunction were recruited as experimental subjects for 8 weeks of wheelchair vibration training intervention before and after wheelchair vibration training. Each of the subjects was tested by CON-TREX isokinetic tester with WIN-POD static balance instrument, and was tested by TUGX 10MWT and 6MWT simultaneously. All the tests were completed by the same staff. After the training, they were compared with each other before and after the training. Results: after 8 weeks of wheelchair vibration training, the knee, ankle flexion and extensor muscle group of the subjects were significantly improved. At 60 掳/ s and 180 掳/ s, the peak torque of extensor muscle of knee joint increased significantly (P 0.05), and the ratio of peak torque of flexion and extensor muscle of knee joint was also significantly different (P 0.05). The ankle joint and knee joint were consistent, and the extensor muscle group of ankle joint was slightly higher than flexor muscle group of ankle joint. There was significant difference in peak torque of extensor muscle of ankle joint (P 0.05), but there was no significant difference in peak torque ratio of ankle joint in flexion and extensor muscle. There was significant difference in standing index of eyes closed eyes (P0.05T) and significant difference in the test results of TUGT and 6MWT (P0.01 / 10MWT). Ankle muscle strength, indicating that wheelchair vibration training can be used as a safe and feasible training method for lower limb motor dysfunction. So it needs further research. Wheelchair vibration training can significantly improve the test results of closed eyes bipedal standing, which shows that wheelchair vibration training can improve the proprioceptive and vestibular function. And effectively improve the lower extremity motor dysfunction of the body control ability. The dynamic balance is also greatly improved compared with before and after the experiment. Wheelchair vibration training has a positive effect on the balance function of middle-aged patients with motor dysfunction of lower extremities, so we think that wheelchair vibration training can be used for routine rehabilitation training of patients with lower extremity dyskinesia.
