四川民族地区县级医院卫生人力资源现状调查及对策研究

【摘要】:在区域经济一体化的背景下,民族地区医疗事业发展是促进区域协调发展的重要措施之一。当前我国民族地区卫生人力资源稀缺成为影响民族区域经济发展的重要问题,加强民族地区卫生人力资源的研究将是解决民族卫生服务结构性矛盾的关键。四川省是一个多民族的大省,其中甘孜藏族自治州属于四川省的地级自治州,管辖18个县域,人口约112万人,以藏族为主,比例为77.8%。四川省处于中国的西南,属于西部区域,社会经济发展与东部地区存在很大的差距,特别是一些地处偏远、交通不便等区域的少数民族,其经济发展更加落后,而以经济为支撑的医疗卫生处于较低的水平。其中卫生人力资源配置不足是造成其医疗卫生水平较低的核心因素之一。本文的研究意义在于有利于丰富我国民族地区卫生人力资源研究理论,为政府、医院等部门完善和加强民族地区人力资源工作做出理论指导;有利于促进我国民族地区卫生事业的发展,构建一支高素质的卫生人力队伍。本文在研究中主要采用了文献综述法、问卷调查法、访谈法、案例分析法,确定本文调查区域为四川省甘孜藏族自治州统辖的18个县,调查对象四川省甘孜藏族自治州各县级医院的卫生人员,以各县广大的人民群众。结合笔者多年的工作经验,开展对四川甘孜藏族自治州各县少数民族地区的卫生人力资源状况的调查,以期为少数民族聚居地区的卫生事业建设和发展提供一定的理论依据。本文的研究核心在于以甘孜藏族自治州为样本,进行四川民族地区县级医院卫生人力资源现况调查。从经济基础、人口和城市发展、卫生机构概况、工作满意度问卷抽样调查、卫生工作人员反映的问题等几个方面,探索四川民族地区县级医院卫生行业环境概述;进行县级医院卫生人力资源配置状况和评价,分析卫生人力资源配置状况、利用每万人口配置均量的差异和每万区域面积配置均量的差异指标来分析县级医院卫生人力资源分布公平性状况,进行县级医院卫生人力资源利用效率的对比。最后对四川甘孜藏族自治州人力资源规模和结构、人力资源素质和质量、地区分布平衡性、卫生人才培育状况、医院规模问题、卫生资源利用、医患关系等几个方面进行分析和讨论,得出相应的对策。
[Abstract]:Under the background of regional economic integration, the development of medical service in minority areas is one of the important measures to promote the coordinated development of regions. At present, the scarcity of health human resources in ethnic minority areas has become an important problem affecting the economic development of ethnic regions. Strengthening the study of health human resources in ethnic minority areas will be the key to solving the structural contradiction of national health services. Sichuan Province is a large multi-ethnic province, of which Ganzi Tibetan Autonomous Prefecture belongs to the prefectural autonomous prefecture of Sichuan Province and has jurisdiction over 18 counties. The population is about 1.12 million, mainly Tibetans, and the proportion is 77.8. Sichuan Province lies in the southwest of China and belongs to the western region. There is a big gap between social and economic development and the eastern region, especially for the ethnic minorities in some remote and inaccessible areas. Its economic development is even more backward. However, the medical and health care supported by economy is at a low level, among which the insufficient allocation of health human resources is one of the core factors contributing to the low level of medical and health care. The significance of this study is to enrich our nation. Research Theory of Regional Health Human Resources, To provide theoretical guidance for the government, hospitals, and other departments to improve and strengthen the work of human resources in minority areas, which is conducive to promoting the development of health services in minority areas in China. In this paper, literature review, questionnaire survey, interview and case analysis were used to determine that the investigation area was 18 counties under the jurisdiction of Ganzi Tibetan Autonomous Prefecture in Sichuan Province. The subjects of the survey were health workers in various county-level hospitals in Ganzi Tibetan Autonomous Prefecture, Sichuan Province, and the broad masses of the people in each county. Combined with the author's many years of work experience, To investigate the situation of health human resources in minority areas in the counties of Ganzi Tibetan Autonomous Prefecture, Sichuan, In order to provide a certain theoretical basis for the construction and development of health services in the areas inhabited by ethnic minorities, the core of this paper is to take Ganzi Tibetan Autonomous Prefecture as a sample. Carry out a survey on the present situation of health human resources in county-level hospitals in Sichuan ethnic areas. From the aspects of economic base, population and urban development, general situation of health institutions, job satisfaction questionnaire, problems reflected by health workers, etc. This paper probes into the overview of the health industry environment in the county hospitals in Sichuan ethnic minority areas, carries out the situation and evaluation of the allocation of the health human resources in the county hospitals, and analyzes the situation of the allocation of the health human resources. The distribution of health human resources in county-level hospitals was analyzed by using the difference index of the average allocation of population per 10,000 population and the average allocation of area per 10,000 regions, and the distribution of health human resources in county-level hospitals was analyzed. Finally, the scale and structure of human resources, the quality and quality of human resources, the balance of regional distribution, the status of cultivation of health personnel and the scale of hospitals in Ganzi Tibetan Autonomous Prefecture of Sichuan Province were compared. The utilization of health resources and the relationship between doctors and patients were analyzed and discussed, and the corresponding countermeasures were obtained.
