【摘要】：大批的农村基层医疗卫生机构在新农合政策的实施下得到极大地激活,进而提高了农村基层医疗卫生服务的水平。现如今位于乡镇的卫生院(在新农合制度设计中处于枢纽地位的乡镇卫生院)和农村卫生室的作用,在国家层面得到政府极大地重视,从而加速了基层医疗卫生机构的建设与发展。基层医疗卫生服务中的农村医务室主要承担所在行政村的一般疾病的诊治工作,是农村居民日常就医的首要途径,农村基层医疗卫生服务机构能否有效地分流农村患者,是农村三级医疗网发挥其价值的关键,农村居民寻医问诊,是否首选基层医疗卫生机构是其发展的首要环节。然而医疗卫生资源城乡配置的失衡,资源利用不公平现象等加剧了乡村两级医疗服务机构专业人才紧缺、医疗设备配备不完善、缺乏先进医疗技术的引进等一系列问题的出现,单靠基层医疗制度的运行不能达成促进基层医疗卫生服的发展目标。文章旨在进一步提高我国基层医疗卫生服务而进行的相关基层调研工作,在充分学习研究了现今我国以及国外学者的相关研究的基础上,以社会医学与卫生事业管理相关的卫生工作全社会参与的观点、健康保险需求理论、公共物品与外部性理论等为基础,对山西洪洞县的基层农村医疗卫生服务现状做深入调研。基于此条件下,首先从基层医疗卫生服务发展背景出发,分析了山西省基层医疗卫生服务各方面实施发展的现状、洪洞县基层医疗卫生服务实施发展的现状。其次对文章研究的相关概念进行了界定,在相关理论基础的指导下,采取入户调查问卷的形式对山西洪洞县农村基层医疗卫生服务进行调研,问卷内容包括:目标农户的相关基本信息、农户对相关政策制度的熟悉程度、基层医疗卫生服务者的农户评价、农户对当地新农合政策实施方面的评价等,进行了定性与定量的分析,在此基础之上,利用相关统计学手段对调研所得数据进行了相关性的单因素和多因素的分析,对所得分析结果一一阐述,发现现行农村基层医疗卫生服务方面在实施过程中存在的问题,以及探讨了相关问题的发生机制。最后结合研究区域经济、社会等因素提出一些适应当地的发展基层医疗卫生服务水平的政策和建议。文章是以山西省洪洞县的几个典型农村作为研究对象,为我国基层医疗卫生服务水平的提高与发展,提供经过实践调研分析得来的相关指导与参考。
[Abstract]:A large number of rural primary medical and health institutions have been greatly activated under the implementation of the new rural cooperative policy, thus improving the level of rural primary medical and health services. At the national level, the role of the township hospitals (which are pivotal in the design of the NCMS system) and the rural health clinics has been greatly valued by the government at the national level. Thus speeding up the construction and development of primary medical and health institutions. Rural medical clinics in primary medical and health services are mainly responsible for the diagnosis and treatment of general diseases in their administrative villages, which is the primary way for rural residents to seek medical treatment on a daily basis. Can rural primary medical and health services effectively divert rural patients? It is the key to give full play to the value of rural three-level medical network. It is the primary link for rural residents to seek medical advice and choose primary medical and health institutions. However, the imbalance of urban and rural allocation of medical and health resources and the unfair utilization of resources have aggravated the shortage of professionals in medical service institutions at the rural level, the imperfect medical equipment, the lack of the introduction of advanced medical technology, and so on. The primary health care system alone can not achieve the goal of promoting the development of primary health care services. This article aims to further improve the primary health care services in China and carry out research work at the grass-roots level, on the basis of a full study of the relevant research of domestic and foreign scholars, Based on the viewpoint of the whole society participation in the health work related to the management of social medicine, the theory of the demand for health insurance, the theory of public goods and externality, the present situation of the primary rural medical and health service in Hongdong County, Shanxi Province, was investigated deeply. Based on this condition, firstly, from the background of the development of primary medical and health services, this paper analyzes the current situation of the implementation and development of primary medical and health services in Shanxi Province, and the present situation of the implementation and development of primary medical and health services in Hongdong County. Secondly, the paper defines the relevant concepts of the research, under the guidance of the relevant theoretical basis, taking the form of household questionnaire to investigate the rural primary medical and health services in Hongdong County, Shanxi Province. The contents of the questionnaire include: the basic information of the target farmers, the familiarity of the farmers with the relevant policies and systems, the farmers' evaluation of the primary medical and health service providers, the evaluation of the farmers' implementation of the local NCMS policy, and so on. On the basis of qualitative and quantitative analysis, this paper analyzes the correlation between single and multiple factors by using the relevant statistical methods, and expounds the results of the analysis one by one. The problems existing in the implementation of rural primary medical and health service are found and the mechanism of the related problems is discussed. Finally, combined with the study of regional economy and social factors, some policies and suggestions are put forward to adapt to the local development of primary medical and health service level. This article takes several typical rural areas in Hongdong County, Shanxi Province as the research object, and provides the relevant guidance and reference for the improvement and development of the primary medical and health service level of our country through the practice investigation and analysis.
