肺癌患者血清p16、FHIT、APC基因甲基化检测

【摘要】:目的:检测肺癌患者血清中抑癌基因p16、FHIT、APC的甲基化,探讨它们在肺癌发生和诊断中的临床价值。方法:收集120例原发性肺癌患者(肺癌组)、46例肺良性疾病患者(对照组)的血液标本,采用甲基化PCR方法检测2组患者血清p16、FHIT、APC基因甲基化。采用logistic回归分析肺癌发生的危险因素,计算上述基因甲基化诊断肺癌的准确率。结果:肺癌组血清中p16、FHIT、APC的甲基化率分别为37.5%、34.2%、31.7%,对照组中分别为6.5%、13.0%、10.9%,差异有统计学意义(P均0.05)。Logistic回归分析提示p16、FHIT基因甲基化均为肺癌发生的独立危险因素,OR(95%CI)分别为7.509(2.160~26.099)、2.927(1.096~7.814)(P均0.05)。p16、FHIT、APC基因甲基化单项诊断肺癌的准确率分别为53%、49%、47%,3种基因甲基化联合诊断肺癌的准确率为87%,优于单项指标。结论:p16、FHIT基因甲基化均为肺癌发生的独立危险因素;联合检测p16、FHIT、APC甲基化可提高肺癌诊断的准确性。
[Abstract]:Objective: To detect the methylation of p16, FHIT and APC in the serum of patients with lung cancer and to explore their clinical value in the occurrence and diagnosis of lung cancer. Methods: The blood samples of 120 patients with primary lung cancer (lung cancer) and 46 patients with benign lung disease (control group) were collected. The methylation of p16, FHIT and APC gene was detected by methylation PCR. The risk factors of lung cancer were analyzed by logistic regression, and the accuracy of the above-mentioned gene methylation in the diagnosis of lung cancer was calculated. Results: The methylation rates of p16, FHIT and APC in the serum of the lung cancer group were 37.5%, 34.2%, 31.7%, and 6.5%, 13.0% and 10.9% in the control group, respectively (P <0.05). Logistic regression analysis suggested that the methylation of p16 and FHIT gene was an independent risk factor for lung cancer, OR (95% CI) was 7.509 (2.160-26.099), 2.927 (1.096-7.814) (P <0.05). The accuracy of p16, FHIT and APC gene methylation in the diagnosis of lung cancer was 53%,49% and 47%, respectively. Conclusion: The methylation of p16 and FHIT is an independent risk factor for lung cancer, and the detection of p16, FHIT and APC methylation can improve the accuracy of the diagnosis of lung cancer.
