非特异性下腰痛个体化运动疗法APP程序初步设计

【摘要】:研究目的:为研发一款非特异性下腰痛(NLBP)个体化运动疗法APP进行初步的程序设计,验证个体化的运动疗法方案对非特异性下腰痛的治疗效果及程序的可行性。研究方法:1.通过查阅大量文献及相关书籍归纳总结成动作库,并设计运动疗法定制方案。2.选取非特异性下腰痛患者30人,随机分为实验组和对照组,各15人。在治疗前对所有受试者进行日常生活正确姿势的指导。实验组由测试人员模拟APP程序制定个体化的运动疗法方案;对照组则采用传统的运动疗法。两组锻炼的时间均为6周,每周5天,每天20-25分钟。分别在治疗前、治疗3周后及治疗6周后进行测评,测评得到的数据采用SPSS19.0软件进行统计学分析。3.查阅APP制作相关文献并研究移动医疗类APP,初步设计出非特异性下腰痛个体化运动疗法APP的基本功能及界面模块。研究结果:1.归纳总结形成动作库,并根据腰椎活动度、肌肉柔韧性、肌耐力及平衡功能的评定将所有动作分类。2.通过模拟APP程序可以实现非特异性下腰痛个体化运动疗法的定制,实验结果如下:(1)VAS视觉模拟评分:实验组在3周及6周较治疗前均有显著性差异(P0.01),疼痛评分明显下降,且实验组较对照组有非常显著性差异(P0.01)。(2)Roland-Morris功能障碍评分:治疗3周及6周后两组较本组治疗前均有非常显著性差异(P0.01),3周时两组之间无显著性差异,6周时两组之间有非常显著性差异(P0.01)。(3)腰椎活动度:两组治疗3周及6周后本组较治疗前均有非常显著性差异(P0.01),两组间在治疗3周时左右侧屈出现显著性差异(P0.05),6周时两组之间在各方向的活动度均出现显著性差异(P0.05)。(4)肌肉柔韧性:实验组在治疗3周及6周后较治疗前均存在非常显著性差异(P0.01),对照组在治疗后与治疗前无显著性差异(P0.05),6周时两组之间存在非常显著性差异(P0.01),实验组明显优于对照组。(5)肌耐力测试:两组治疗后较治疗前均存在非常显著性差异(P0.01),且实验组比对照组有更显著的提高作用,治疗后较治疗前存在显著性差异。(6)稳定性测试:两组治疗3周及6周后与治疗前均有非常显著性差异(P0.01),但两组之间无显著性差异(P0.05)。3.APP程序初步设计完成,基本功能模块及界面已确定。研究结论:非特异性下腰痛个体化运动疗法APP可以实现患者定制个体化的运动疗法方案、督促患者坚持锻炼并向大众普及腰痛知识。个体化的运动疗法可以有效缓解NLBP患者的疼痛程度、显著改善生存质量、增加腰椎活动度、改善柔韧性、提高核心耐力、改善平衡能力,能多维度提高NLBP患者的核心稳定性、缓解症状、提高生存质量。
[Abstract]:Objective: to design a program for the development of a non-specific low back pain (NLBP) individual exercise therapy (APP). To verify the feasibility of individualized exercise therapy in the treatment of non-specific low back pain. And designed a customized exercise therapy program .2. 30 patients with non-specific low back pain were randomly divided into experimental group and control group. Before treatment, all subjects were given correct posture instruction in daily life. In the experimental group, the individual exercise therapy program was developed by simulating the APP program. The control group was treated with traditional exercise therapy. The exercise time of the two groups was 6 weeks, 5 days a week, 20-25 minutes per day, respectively before treatment, 3 weeks after treatment and 6 weeks after treatment. The data were analyzed by SPSS19.0 software. 3. The basic function and interface module of APP for individual exercise therapy of non-specific low back pain were preliminarily designed by consulting the related documents of APP and studying the mobile medical app. The result is: 1.Induction and summary form the action database, According to the evaluation of lumbar motion, muscle flexibility, muscle endurance and balance function, all the movements were classified. 2. The customization of individual exercise therapy for non-specific low back pain can be realized by simulating APP program. The results were as follows: the visual analogue score of VAS in the experimental group was significantly different from that before treatment at 3 weeks and 6 weeks, and the pain score was significantly decreased. The scores of Roland-Morris dysfunction in the experimental group were significantly higher than those in the control group. There was no significant difference between the two groups after 3 weeks and 6 weeks of treatment compared with that before treatment. There was no significant difference between the two groups at 6 weeks after treatment. Significant difference in lumbar movement: there was a significant difference between the two groups after 3 weeks and 6 weeks of treatment than before treatment. There was a significant difference between the two groups in the left and right side flexion at 3 weeks after treatment, and there was a significant difference between the two groups at 6 weeks after treatment. There was significant difference in muscle flexibility in the experimental group after 3 weeks and 6 weeks compared with that before treatment (P 0.01), and there was no significant difference between the control group and the control group after treatment and before treatment at 6 weeks after treatment (P 0. 05 and P 0. 05%) (P < 0. 05), and there was no significant difference between the control group and the control group at 6 weeks after treatment (P < 0. 05). There was a very significant difference between the two groups. The experimental group was obviously superior to the control group in muscle endurance test. There was a very significant difference between the two groups after treatment than before, and the experimental group had a more significant improvement than the control group. There was significant difference in stability between the two groups after treatment 3 weeks and 6 weeks after treatment (P 0.01), but there was no significant difference between the two groups. The basic functional modules and interfaces have been determined. Conclusion: APP can be used to customize individual exercise therapy for patients with low back pain. The individualized exercise therapy can effectively alleviate the pain degree of NLBP patients, significantly improve the quality of life, increase lumbar mobility, improve flexibility, improve core endurance. Improving the balance ability can improve the core stability, alleviate symptoms and improve the quality of life of patients with NLBP.
