高频超声检测糖尿病周围神经病患者足部肌肉和足底组织的变化

【摘要】:目的:探讨高频超声在检测2型糖尿病(T2DM)合并周围神经病变(DPN)患者足部肌肉和足底组织变化中的应用价值。方法:选取住院的T2DM患者60例,并分为两组:T2DM合并DPN患者为DP组,30例;T2DM未合并DPN患者为DM组,30例。另收集健康志愿者25例作为对照(ND组)。应用高频超声检查受检者非优势足的趾短伸肌(EDB),第一、二跖趾间肌群(MIL)以及第一至第五跖骨头下足底皮肤(表皮和真皮)、足底脂肪组织,测量EDB的厚度与横截面,MIL及跖骨头下足底皮肤、足底脂肪组织的厚度。应用t检验和秩和检验进行数据分析,以P0.05为差异有统计学意义。结果:DP组的EDB的厚度和横截面积以及MIL的厚度较DM组、ND组均明显减小(P均0.01); DM组的EDB横截面积以及MIL厚度较ND组均明显减小(P均0.01);DP组的EDB厚度和横截面积以及MIL厚度较DM组均明显减小于(P均0.01)。DP组、DM组的足底皮肤厚度较ND组均减小(P均0.05)。DP组的足底脂肪组织厚度较DM组、ND组均减小(P均0.05), DM组的足底脂肪组织厚度较ND组均减小(P均0.05)。结论:高频超声检查能够检测到2型糖尿患者足部肌肉萎缩以及足底皮肤和脂肪组织变薄;2型糖尿病患者合并周围神经病变后肌肉萎缩和足底脂肪组织变薄程度加重。高频超声检查可以客观的显示T2DM及合并DPN患者的足部肌肉萎缩和足底组织变薄。
[Abstract]:Objective: to investigate the value of high frequency ultrasound in detecting the changes of muscle and plantar tissue in patients with type 2 diabetes mellitus (T2DM) complicated with peripheral neuropathy (DPN). Two groups were divided into two groups: DP group (n = 30) with T2DM and DPN (n = 30) and healthy volunteers (n = 25) as control group (n = 30). High frequency ultrasound was used to examine the extensor digitorum brevis muscle of non-dominant foot. The thickness and cross section of EDB and the skin of the first to 5th metatarsal heads (epidermis and dermis, plantar adipose tissue) and the subplantar skin of the first to 5th metatarsal heads were measured. Thickness of plantar adipose tissue. Data analysis using t test and rank sum test. Results the thickness and cross-sectional area of EDB and the thickness of MIL in the group of EDB were significantly decreased than those in the group of DM and ND, and the cross-sectional area of EDB and the thickness of MIL in the group of DM were significantly lower than those in the group of ND, and the thickness of MIL in the group of DM was significantly lower than that in the group of ND. The thickness of EDB, cross sectional area and MIL in DM group were significantly lower than those in DM group. The thickness of plantar skin in DM group was significantly lower than that in ND group. The thickness of plantar adipose tissue in DM group was lower than that in ND group, and the thickness of plantar adipose tissue in DM group was lower than that in ND group, and the foot thickness in DM group was lower than that in DM group. Conclusion: high frequency ultrasound can detect foot muscle atrophy and skin and adipose tissue thinning in type 2 diabetic patients with peripheral neuropathy. High frequency ultrasound could objectively show the muscle atrophy and the thinning of plantar tissue in patients with T2DM and DPN.
