3.0T心脏磁共振稳态自由进动序列测量正常汉族中国人主动脉根部:可行性、参考值范围及影响因素

【摘要】:目的:采用3.0T CMR的稳态自由进动序列(Steady-state free precession,SSFP)对正常汉族中国人主动脉根部的形态学指标进行测量,评价其可行性,提出适合不同年龄段、不同性别正常汉族中国人的参考值范围,并分析使得主动脉根部各指标变化的影响因素。方法:纳入健康志愿者100名(20-70岁),分为5组(20~29岁组;30~39岁组;40~49岁组;50~59岁组;60~69岁组),每组男性和女性各10名。采用SSFP序列进行3.0T CMR矢状位(Sagittal)和冠状位(Coronal)左室流出道(left ventricular outflow tract,LVOT)的电影图像扫描,于最大收缩期和舒张末期层面分别测量主动脉瓣环水平、主动脉窦中部水平以及窦管交界水平的径线值,比较各径线值之间的差异。结果:共计扫描志愿者106人,最终纳入100名志愿者,男女各50名,平均年龄为44.36±13.82岁。本研究测得了不同年龄段、不同性别正常汉族中国人的参考值范围。收缩期与舒张期主动脉根部各径线测量值之间存在差异,如男性收缩期、舒张期在矢状位瓣环水平的测量值分别为22.11±1.65 mm、21.37±1.84 mm,P0.05;女性收缩期、舒张期在矢状位瓣环水平的测量值分别为20.87±1.45 mm、20.04±1.36 mm,P0.05。男性与女性主动脉根部各径线测量值之间存在差异,如男性和女性在矢状位瓣环水平收缩期测量值分别为22.11±1.65 mm、20.87±1.45 mm,P0.05;舒张期相应测量值分别为21.37±1.84 mm、20.04±1.36 mm,P0.05。冠状位与矢状位主动脉根部各径线测量值之间存在差异,如冠状位和矢状位瓣环水平男性的收缩期测量值分别为23.14±1.72 mm、22.11±1.65 mm,P0.05;舒张期相应测量值分别为22.34±1.94 mm、21.37±1.84 mm,P0.05。不同年龄段主动脉根部各径线测量值之间存在差异,如20-29岁组、30-39岁组、40-49岁组、50-59岁组、60-69岁组矢状位瓣环水平男性的收缩期测量值分别为21.24±1.28 mm、21.98±1.55 mm、22.11±1.44 mm、22.59±1.48 mm、22.61±1.33 mm,P0.05。不同水平主动脉根部各径线测量值之间存在差异,如瓣环水平、窦瓣结合部水平、主动脉窦水平男性矢状位收缩期测量值分别为22.11±1.65 mm、24.24±1.75 mm、30.81±1.94mm,P0.05。按照身高和体表面积标化后,上述结果仍然成立(P0.05)。主动脉根部各径线值与年龄之间呈正相关,随着年龄的增加,主动脉根部各径线值亦增加(P0.05)。体表面积较身高对主动脉根部各径线值的影响更大,且对男性志愿者的舒张期径线值的影响更大(P0.05)。结论:1.3.0T CMR的SSFP序列可用于正常汉族中国人主动脉根部的评价。2.在对主动脉根部进行形态学评价时,需要综合考虑性别、年龄、身高、体表面积、心脏运动、不同径线水平以及成像层面等因素对主动脉根部各测量值的影响。
[Abstract]:Objective: to measure the morphological indexes of the aortic root of normal Han Chinese by using the Steady-state free precession (SSFP) sequence (Steady-state free precession, SSFP), to evaluate the feasibility of the aortic root of the Chinese Han people of the normal Han nationality, and to put forward the range of the reference value of the Chinese people of the normal Han people of different ages and different sexes, and to analyze the changes of the indexes of the root of the aorta. Methods: 100 healthy volunteers (20-70 years old) were divided into 5 groups (20~29 year old group, 30~39 year old group, 40~49 year old group, 50~59 year old group, 60~69 year old group), 10 men and women in each group. Conclusion: the SSFP sequence of 1.3.0T CMR can be used to evaluate the aortic root of the normal Han Chinese people when.2. is used for the morphological evaluation of the root of the aorta. The effects of sex, age, height, body surface area, heart movement, different diameter levels and imaging level on the measurement of aortic root values need to be considered comprehensively.
