背心式高频振动排痰与机械振动排痰对老年肺部感染患者的效果观察

【摘要】:目的探讨背心式高频振动排痰和机械振动排痰对老年肺部感染患者的疗效对比。方法选取30例老年肺部感染患者,随机分为高频组和机械组各15例。在常规治疗基础上,高频组采用背心式高频振动排痰仪,机械组采用机械振动排痰仪。观察两组在治疗前(D1)、治疗后第7 d(D7)、第14 d(D14)的呼吸频率(R)、末梢静脉氧饱和度(SpO_2)、日常生活能力(ADL)评分、住院时间、痰液情况等指标。结果两组日排痰量无明显差异,同一组中D7比D1,D14比D7的日排痰量比较,差异均有统计学意义。D7、D14的SpO_2、ADL评分比较,高频组高于机械组,差异有统计学意义(P0.05)。组内比较,D7和D14分别与D1比较,R、ADL、SpO_2均有明显变化,差异有统计学意义(P0.05)。D14与D7比较,除了机械组的ADL无明显差异外,其余各指标比较,差异均有统计学意义(P0.05)。结论两种振动排痰方法均有效,痰量变化无差异,背心式高频振动排痰仪能较长时间改善咯痰难易度和痰液性状。该法能锻炼胸廓收缩力,提高血氧、缓解气促。
[Abstract]:Objective to compare the efficacy of vest high frequency vibration sputum drainage and mechanical vibration sputum drainage in elderly patients with pulmonary infection. Methods 30 elderly patients with pulmonary infection were randomly divided into high frequency group (15 cases) and mechanical group (15 cases). On the basis of routine treatment, the high frequency vibration sputum ejector was used in the high frequency group and the mechanical group was used. The respiratory rate (R), peripheral venous oxygen saturation (SPO _ 2), ADL score, hospitalization time and sputum were observed before treatment (D1), 7th day (D7), 14th day (D14). Results there was no significant difference in daily sputum excretion between the two groups. In the same group, the daily sputum excretion volume of D7 was higher than that of D1D14, the difference of SPO2ADL of D7D14 was statistically significant, and the high frequency group was higher than that of mechanical group (P0.05). The difference between D7 and D14 and D1 was statistically significant (P0.05) .D14 and D7, except for the mechanical group, there was no significant difference in ADL between the two groups (P0.05). Conclusion both of the two methods are effective and there is no difference in sputum volume. High frequency vibration sputum ejector with vest type can improve the easy degree of sputum and sputum properties for a long time. This method can exercise chest contractility, improve blood oxygen and relieve shortness of breath.
